角膜基质透镜联合羊膜移植术治疗真菌性角膜溃疡

摘　　要：目的观察角膜基质透镜联合羊膜移植术治疗真菌性角膜溃疡的临床效果。方法回顾性病例系统研究。搜集2015年1月至2015年12月真菌性角膜溃疡14例（14只眼）的临床资料，患者均有植物性外伤史。采用角膜基质透镜联合羊膜移植术治疗。角膜基质透镜来自于接受全飞秒屈光手术的近视者，所有捐献者均知情同意及做血液传染病检查。取出的角膜基质材料置于消毒的纯甘油，-80℃低温冰箱保存备用。结果14例真菌性角膜溃疡炎症消退，角膜溃疡痊愈。13例一次手术成功，1例二次手术成功。周边部角膜溃疡病例愈合后患者恢复较好视力。结论角膜基质透镜联合羊膜移植术治疗真菌性角膜溃疡安全有效。Objective To observe the clinical effect of corneal stroma lenticule combined with amniotic membrane transplantation for fungal corneal ulcer. Methods A retrospective study was performed. The data of 14 eyes of 14 patients with fungal corneal ulcer was collected from January 2015 to December 2015. All patients had histories of plant ocular trauma. Corneal stroma lenticule combined with amniotic membrane transplantation were performed for fungal corneal ulcer. Corneal stroma lenticules were taken from myopia patients who underwent femtosecond laser-assisted small incision lenticule extraction. For all these donors, informed consent was obtained and blood-borne diseases tests were performed. Extracted corneal stroma lenticules were placed into sterilized pure glycerin in a refrigerator at -80 ~C. Results Inflammation of fungal corneal ulcer disappeared and cornea ulcer well healed in all the 14 cases. Thirteen patients underwent 1 operation and 1 patient underwent 2 operations until corneal ulcers healed. Patients with peripheral corneal ulcer had better vision after ulcer healed. Conclusion Corneal stroma lenticule combined with amniotic membrane transplantation for fungal corneal ulcer is safe and effective.
