复发性抑郁障碍危险因素分析

摘　　要：目的 探讨复发性抑郁障碍的危险因素. 方法 对302例抑郁患者进行跟踪随访,将复发者设为研究组,未复发者设为对照组,采用自制调查表统计两组患者的一般资料,采用社会支持评定量表、简易应对方式问卷、家庭功能评定问卷评定两组的社会心理因素,对抑郁复发的影响因素进行回归分析.结果 两组疾病遗传史及治疗依从性比较差异有统计学意义（P〈0.05或0.01）.研究组简易应对方式问卷积极应对评分显著低于对照组（P〈0.01）,消极应对评分显著高于对照组（P〈0.01）,社会支持评定量表总分及对支持的利用度评分均显著低于对照组（P〈0.01）,家庭功能评定问卷情感反应及情感介入评分均显著低于对照组（P〈0.01）.治疗依从性、遗传史、情感反应、情感介入为抑郁症复发的危险因素.结论 服药依从性、遗传史、情感反应、情感介入为抑郁症复发的危险因素,应予以重视,预防患者复发.Objective To explore the risk factors of recurrent depressive disorder.Methods A total of 302 depression patients were followed up, recurrent patients assigned to research group, non-recurrent ones to control group, general data of both groups added up, psychosocial factors assessed with the Social Support Rating Scale （SSRS）, Simplified Coping Style Questionnaire （SCSQ） and Family Assessment Device （FAD）, and regression analyses conducted for influencing factors of depression relapse.Results There were statistical significance in group differences of genetic history and treatment compliance （P〈0.05 or 0.01）.Active coping score of the SCSQ was significantly lower （P〈0.01）, negative coping higher （P〈0.01）, total and support availability of the SSRS were lower （P〈0.01）, and so were affective responsiveness and affective involvement of the FAD （P〈0.01） in research than in control group.Risk factors of depression relapse were treatment compliance, genetic history, affective responsiveness and affective involvement.Conclusion Treatment compliance, genetic history, affective responsiveness and affective involvement are risk factors of depression relapse, attention should be paid to the factors to prevent patients＇ relapses..
