四黄清胃汤联合四联疗法根除幽门螺杆菌(HP)随机平行对照研究  

Four Huang Qingwei Decoction Combined with Quadruple Therapy for Helicobacter Pylori(HP) Infection in a Randomized Controlled Trial

摘　　要：[目的]观察四黄清胃汤联合四联疗法根除幽门螺杆菌(HP)疗效。[方法]使用随机平行对照方法,将70例住院及门诊患者就诊顺序号方法简单随机分两组。对照组35例四联疗法(阿莫西林+克拉霉素+泮托拉唑钠+枸橼酸铋钾)。治疗组35例四黄清胃汤(黄芪15g,川连6g,黄芩、黄柏各10g,乌梅、公英各12g,厚朴9g,党参15g,丹参12g,山楂、白术各15g,炙甘草6g),随症加减,1剂/d,水煎150m L,早晚口服;四联疗法同对照组。连续治疗14d为1疗程。观测临床症状、幽门螺杆菌(HP)、不良反应。治疗1疗程,根除治疗4周,判定疗效。随访3个月、6个月,观测不良反应及幽门螺杆菌(HP)。[结果]治疗组根除31例,未根除3例,根除率91.18%;对照组根除27例,未根除7例,根除率79.41%;治疗组疗效优于对照组(P<0.05)。随访3个月、6个月,未见不良反应发生及13C-UBT检测仍为阴性。[结论]四黄清胃汤联合四联疗法根除幽门螺杆菌(HP),疗效满意,无严重不良反应,值得推广。[Objective] To observe four Huang Qingwei decoction combined with quadruple therapy for Helicobacter pylori （Hp） infection curative effect. [Method] Using random parallel control method, 70 cases of inpatients and outpatients were randomly divided into two groups. The control group of 35 cases of amoxicillin ＋elarithromyein＋pantoprazole sodium＋bismuth potassium citrate, quadruple therapy. The treatment group of 35 cases four Huang Qingwei decoction （astragalus 15g, kawatsure 6g, Scutellaria, phellodendron, 10g, fructus mume, dandelion 12g, magnolia 9g, dangshen 15g, salvia 12g, hawthorn, atractylodes each 15g, zhigancao 6g） with disease, 1 agent/d, 150mL oral decoction, sooner or later; four treatment with the control group. Continuous treatment for 1 courses of 14d. Clinical symptoms, 13C-UBT, adverse reactions were observed. After 1 weeks of treatment, the curative effect was determined after eradication treatment for 4 weeks. All the patients were followed up for 3 months and 6 months. Adverse reactions and 13C-UBT. [Results] the treatment group 31 cases were cured, 3 cases were ineffective, the total efficiency of 91.18%; control group 27 cases were cured, 7 cases were ineffective, the total efficiency of 79.41%; The curative effect of the treatment group was better than that of the control group （P〈0.05）. All patients were followed up for 3 months and 6 months, and no adverse reactions were observed and 13C-UBT test was negative. [Conclusion] Four Huang Qingwei decoction combined with quadruple therapy for Helicobacter pylori （Hp） infection, satisfactory curative effect, no serious adverse reactions, worthy of promotion.
