摘　　要：目的探讨胃腺癌中YAP蛋白的表达情况及其与胃腺癌患者临床病理特征及预后之间的关系。方法应用免疫组织化学法检测人胃腺癌组织中YAP蛋白的表达情况,并用Kaplan-Meier单因素生存分析法和Cox模型多因素回归分析法分析YAP蛋白表达与胃腺癌患者的临床病理特征及预后之间的关系。结果胃腺癌组织中YAP蛋白的细胞质阳性表达(31.6%vs 1.0%)、细胞核阳性表达(22.4%vs 0.0%)均明显高于正常胃黏膜组织(P﹤0.01)。YAP蛋白细胞质阳性表达与细胞核阳性表达呈正相关(Spearman相关系数=0.5818,P﹤0.0001)。Kaplan-Meier单因素生存分析结果显示,YAP蛋白细胞核阳性组患者的5年生存率较YAP蛋白细胞核阴性组低(P﹤0.05)。Cox模型多因素回归分析显示,YAP蛋白的表达未能作为胃腺癌患者预后的独立预测因子(P﹥0.05)。结论部分胃腺癌组织中存在YAP蛋白的高表达,且YAP蛋白细胞核表达的胃腺癌患者的预后较差,可以作为此病的早期诊断指标。Objective To explore the expression of YAP protein and its relation to clinicopathological factors and prognosis in patients with gastric adenocarcinoma. Method The expression of YAP protein in gastric adenocarcinoma tissues was measured with immunohistochemical method, and its relation to clinicopathological factors and prognosis in patients with gastric adenocarcinoma was analyzed using Kaplan-Meier univariate analysis and multivariate analysis with Cox proportional hazard model. Result In gastric adenocarcinoma issues, the positive expression of YAP protein was found in the cytoplasm of 31.6%samples and in the nuclei of 22.4%samples, both significantly higher than those in nor-mal gastric mucosa （1.0% and 0.0%, respectively） （P〈0.01）. The positive expression of YAP protein in cytoplasm was positively correlated with the positive expression in nuclei （Sperman correlation coefficient=0.5818, P〈0.0001）. Results of Kaplan-Meier univariate analysis showed that the 5 year survival rate of patients with positive YAP expression in nu-clei was lower than that of patients with negative results （P〈0.05）. The results of multivariate analysis using Cox propor-tional hazard model showed that the expression of YAP protein could not be used as an independent predictor of progno-sis in patients with gastric adenocarcinoma （P〉0.05）. Conclusion Some gastric adenocarcinoma issues have high expres-sion of YAP protein, the nuclear expression of YAP protein and the poor prognosis of gastric adenocarcinoma can be used as an early diagnostic marker for this disease.
