摘  要:目的探讨CO_2激光联合低温等离子射频消融术治疗累及前联合早期声门型喉癌的疗效。方法选取2012年3月至2015年7月间南京医科大学第一附属医院和徐州医科大学附属淮安医院收治的70例累及前联合早期声门型喉癌患者,观察组采用CO_2激光联合低温等离子射频消融术(30例)治疗,对照组采用CO_2激光(40例)治疗。对比两组患者嗓音障碍情况及术后复发率。结果术后3个月,两组患者嗓音障碍指数量表(VHI)等级分布比较,差异无统计学意义(P>0.05);观察组患者复发率为6.7%,对照组为25.0%,两组比较,差异有统计学意义(P<0.05)。结论 CO_2激光联合低温等离子射频消融术治疗累及前联合早期声门型喉癌疗效确切,对病变暴露困难者更有优势,喉部结构和喉功能保全良好。Objective To investigate the efficacy of CO2 laser combined plasma radiofrequency ablation at a low temperature in the treatment of early glottic carcinoma involving the anterior commissure.Methods From March 2012 to July 2015,70 patients with early glottic carcinoma involving the anterior commissure treated at First Affiliated Hospital of Nanjing Medical University and The Affiliated Huaian Hospital of Xuzhou Medical University were selected. Patients in the observation group received CO2 laser combined with radiofrequency ablation therapy(30 patients) and patients in the control group received CO2 laser therapy(40 patients). Voice disorders as well as postoperative recurrence rate were compared between the two groups. Results There was no significant difference in voice handicap index(VHI) scores between the two groups at 3 months after the surery(P〉0. 05). The recurrence rate was significantly less in the observation group(6. 7%) than in the control group(25. 0%)(P〈0. 05). Conclusion CO2 laser combined with low-temperature plasma radiofrequency ablation shows high efficacy for early glottic carcinoma involving the anterior commissure,especially for lesions in difficult laryngeal exposure,which achieves preservation of larynx with best possible laryngeal function.
