MUC1和CK19蛋白在乳腺癌前哨淋巴结术中检测中的应用研究

摘　　要：目的:探讨检测MUC1和CK19蛋白用于术中冰冻联合快速免疫组化法在乳腺癌前哨淋巴结术中诊断中的可行性、应用价值和临床意义。方法:以蓝染法对164例乳腺癌患者实施前哨淋巴结活检(sentinel lymph node biopsy,SLNB),154例成功,对检得的475枚淋巴结施行术中冰冻切片并联合行快速免疫组化检测,以粘蛋白MUC1和细胞角蛋白CK19为检测指标,与金标准术后连续切片为对照。结果:冰冻切片联合快速免疫组化法行SLN术中检测诊断阳性率(26.3%)高于常规冰冻切片(20.2%),差异有统计学意义(P<0.05),而与金标准术后连续切片检出率(27.8%)的差异无统计学意义(P>0.05)。结论:乳腺癌SLN术中诊断中,以MUC1和CK19为指标行术中冰冻切片联合快速免疫组化法检测,可以提高SLN的术中阳性检出率,降低假阴性率,从而减少了假阴性导致的二次手术发生,且安全、快速、花费少,值得在临床推广。Objective ： To investigate the feasibility and application value of intraoperative diagnosis and clinical significance of the protein detection （MUC1） and CK19 used in intraoperative frozen combined rapid immunohistochemical method in breast cancer sentinel lymph node. Methods： With blue staining for 164 cases of patients with breast cancer sentinel lymph node biopsy, 154 cases of suc-cess, to check the 475 lymph nodes of intraoperative frozen section joint rapid immunohistochemical detection, with mucin （MUC1） and cytokeratin GK19 were as test index, comparison with postoperative serial section （ the gold standard）. Result ： The results of fro-zen joint rapid immunohistochemical method positive rate（26.3% ） was higher than conventional frozen section（20.2% ） in SLNS intr-aoperative diagnosis, difference was statistically significant （P 〈0.05） ,while with the gold standard in postoperative serial section de- tection rate （27. 8% ）. The differences of no statistical significance （P〉0.05）. Conclusion： Intraoperative frozen section joint rapid immunohistochemical method used in intraoperative diagnosis of breast cancer sentinel lymph node, with MUC1 and CK19 was as index line improves the SLNS positive rate of intraoperative diagnosis, reduces the false negative rate, there by reducing the false negative re-sult in the second operation,and safe, fast, and spend less is worth popularizing in clinical.
