摘  要:目的探究血清神经元特异性烯醇化酶(NSE)及脑源性神经营养因子(BDNF)与婴儿癫痫发作、发展的关联。方法选取2014年6月至2015年6月邯郸市中心医院门诊及住院收治的发作性疾病婴儿共88例,分为癫痫发作组48例和非癫痫性发作组40例。其中,癫痫发作组未经过治疗者18例,治疗效果良好者16例,治疗效果不佳者14例,比较该3组患者的BDNF水平;采用酶联免疫吸附法(ELISA)检测癫痫发作婴儿、非癫痫性发作婴儿发作性表现后48h血清NSE及BDNF水平,分别与健康体检的正常婴儿45例(正常对照组)进行比较分析。以发作时间5min为界限,将患者分为两组,分别检测NSE和BDNF水平。结果癫痫发作组血清NSE及BDNF水平均明显高于非癫痫发作组和正常对照组,差异有统计学意义(P<0.05);抽搐持续超过5min的患者血清NSE及BDNF均明显高于抽搐时间在5min以下者,差异有统计学意义(P<0.05);未经过治疗组、治疗效果良好组、治疗效果不佳组之间BDNF水平比较,差异无统计学意义(P>0.05)。结论血清NSE及BDNF可作为临床上诊治婴儿癫痫发作、检测疾病严重程度的重要参考指标。Objective To explore the correlation of serum neuron specific enolase(NSE)and brain-derived neurotrophic factor(BDNF)with epileptic seizures and development in infants. Methods 88 infants with paroxysmal disease were selected from the outpatients and inpatients of Handan Central Hospital between June of2014 and June of 2015 and divided into the epileptic seizure group with 48 cases and the non-epileptic seizure group with 40 cases.In the epileptic seizure group,18 cases didn′t receive any treatment,16 cases were treated with good curative effect,and 14 cases were treated with poor curative effect.The levels of BDNF in the three subgroups were compared.The enzyme linked immunosorbent assay(ELISA)method was used to detect the levels of NSE and BDNF in the infants of the two groups 48 hours after the onset of seizures,which were compared with those of the normal control group which consisted of 45 healthy infants respectively.Meanwhile,the levels of NSE and BDNF were detected and compared between the patients with less than 5minutes of seizures and those with no less than 5minutes.Results The levels of serum NSE and BDNF in the epileptic seizure group were significantly higher than those in the non-epileptic seizure and the normal control group(P〈0.05).The levels of NSE and BDNF were significantly higher in the patients with no less than 5minutes of seizures than in those with less than 5minutes of seizures(P〈0.05).There were no significant differences in the levels of BDNF among the patients with no treatment,those with good curative effect and those with poor curative effect(P〉0.05).Conclusion The levels of NSE and BDNF can be considered as important clinical indicators of diagnosing the epileptic seizures and detecting the severity of epilepsy in infants.
