机用ProTaper镍钛锉在磨牙根管治疗中的临床研究

摘　　要：目的评价ProTaper镍钛锉在磨牙根管治疗中的效果。方法选取患牙髓病、根尖周病患者97例,共108颗患牙,随机分两组;1组用ISO不锈钢K锉逐步后退手扩法预备根管（手扩组）,1组用机用ProTaper镍钛锉冠向深入法预备根管（机扩组）;均采用氢氧化钙碘仿糊剂导入根管封药,7~10d后复诊;复诊无症状、无叩痛时采用AH糊剂加牙胶尖侧压充填根管;机扩组用0.04或0.06锥度牙胶尖充填,手扩组用0.02锥度牙胶尖充填。观察比较两组的根管预备时间、根管预备形态、器械折断情况和根管充填效果,并行根管预备术后疼痛评估。结果机扩组的根管预备时间和根管充填时间均短于手扩组,差异均有统计学意义（P〈0.001）;机扩组根管预备形态优于手扩组,器械分离率低于手扩组,差异均有统计学意义（P〈0.001）;机扩组根充恰填率高于手扩组,差异有统计学意义（P〈0.001）;机扩组根管预备后疼痛发生率低于手扩组,差异有统计学意义（P=0.020）。结论 ProTaper机扩在根管治疗效率和效果方面均优于不锈钢K锉手扩。Objective To evaluate the clinical effects of ProTaper NI-TI files in the molar root canal therapy.Methods A total of 108 teeth in 97 cases pulpal and periapical diseases were divided randomly into the K-file group by the common K filing and the NI-TI group by the Protaper NI-TI filing.The calcium hydroxide iodoform paste was imported into the root canal.AH Plus paste with gutta percha was used to fill the root canal in the painless return visits after 7 to 10 days by the lateral pressure obturation method.The NI-TI group was filled with a 0.04 or 0.06 tapered gutta-percha,while the K-file group was filled with a 0.02 tapered gutta-percha.The time and shapes of the root canal preparation,the rate of instrument fracture and the filling effects were compared between the two groups and the pain assessment was conducted after the operation.Results The time of preparing and filling the root canal in the NI-TI group was significantly shorter than that in the K-filing group（P〈0.001）.The shapes of the root canal preparation and the filling effects were significantly better in the NI-TI group than in the K-filing group（P〈0.001）,but the rate of instrument fracture was significantly lower in the NI-TI group than that in the Kfiling group（P〈0.001）,and the postoperative pain occurrence of the NI-TI group was significantly less than that of the K-filing group（P=0.020）.Conclusion The ProTaper NI-TI files have higher efficiency and better quality in the molar root canal therapy than the common K files.
