原发性开角型青光眼的中医分型与RNFL厚度改变及视野损害相关性分析

摘　　要：目的探讨原发性开角型青光眼的中医分型与RNFL厚度改变及视野损害相关性。方法选择2010年3月—2012年3月该院收治的初诊30例P0AG患者,另取30名正常人群作为对照组,予以所有研究对象常规眼科检查以及OCT检查以及视野检查,并进行中医证型判定。结果与对照组上方(142.8±19.8)μm、下方(143.2±19.4)μm、颞侧(93.4±16.5)μm、鼻侧(86.3±19.6)μm以及均值(124.5±13.8)μm比较,实证、虚实夹杂证以及虚证的RNFL厚度测量中各项指标均降低明显,差异具有统计学意义(P<0.05);且与虚症所占比例比较,实证、虚实夹杂证的上方、下方、颞侧、鼻侧、均值、旁中心暗点、弓形及环形暗点均增加明显,弥漫性、中心颞侧视岛残留的比例则降低明显,差异具有统计学意义(P<0.05)。结论 P0AG患者早期视神经损害多见实证,晚期则多见虚证,可根据患者的病情进展予以相应的中医辨证治疗。Objective To investigate the Correlations between the thickness change of retinal nerve fiber layer and visual field damage in the primary open angle glaucom a for the syndrome differentiationo of TCM. Methods Choice of 30 patients with P0 AG admitted in our department,the other 30 cases of normal people as control group from March2010 to March 2012,to all the object of study routine ophthalmologic examination and OCT and visual field examination,and TCM syndrome type. Results And the control group（142.8±19.8）μm,below（143.2±19.4）μm,temporal（93.4±16.5）μm,nasal（86.3±19.6）μm and（mean 124.5±13.8）μm,and RNFL,an actual card thickness measurement threedeficiency indicators were significantly lower. The difference was statistically significant（P〈0.05）; and compared with the disease proportion,empirical,with the actual card above and below,temporal,nasal,mean,paracentral scotoma,segmental and ring scotoma were increased significantly,diffuse,temporal side center residual Island ratio is reduced significantly lower,the difference statistical significance（P〈0.05）. Conclusion The optic nerve damageis more common in the deficiency w ith in the blue wind optic nerve damage were seen in the early period. Therefore the extent of optic nerve damage to a certain extent gives a certain degree of guidance in TCM treatment
