摘 要:目的探讨格林巴利综合征(GBS)早期神经电生理改变特点。方法回顾性分析我院50例确诊为GBS患者发病2周内神经电生理检测结果。结果 20例(40%)患者运动神经传导速度(MCV)异常,15例(30%)患者复合肌肉动作电位(CMAP)异常,共计25例(50%)患者运动神经传导异常;5例(10%)患者感觉神经传导速度异常,4例患者(8%)感觉神经动作电位(SNAP)波幅异常,共计6例(12%)患者感觉神经传导异常;43例(86%)患者F波异常,30例(60%)H反射异常。结果 F波及H反射异常率与运动神经传导异常率差异有统计学意义(P<0.05)。运动神经传导异常率较感觉神经传导异常率差异有统计学意义(P<0.05)。结论 F波及H反射对格林巴利综合征早期诊断具有重要价值。运动神经传导速度检查对格林巴利综合征早期诊断价值高于感觉神经传导速度检查。Objective To investigate the electrophysiological characteristics in patients with early Guillain Barre Syndrome(GBS). Methods The electrophysiological data of the 50 patients with GBS within 2 week in our hospital were retrospectively analyzed. Results 20 cases(40%) of motor nerve conduction velocity(MCV) in patients with abnormal, 15 cases(30%) patients with compound muscle action potential(CMAP) abnormalities, a total of 25 cases(50%) patients with motor nerve conduction abnormalities; 5 cases(10%) patients with sensory nerve conduction velocity abnormalities, 4 patients(8%) sensory nerve action potential(SNAP) amplitude abnormalities, a total of 6 cases(12%) patients with sensory nerve conduction abnormalities; 43 cases(86%) patients with abnormal F wave, 30 cases(60%) patients with abnormal H reflex. The abnormal rate of the F waves and H reflex was significantly different from that of motor nerve conduction(P〈0.05). The abnormal rate of motor nerve conduction was significantly different from that of sensory nerve conduction(P〈0.05). ConclusionThe F waves and H-reflex are importantly valuable in the diagnosis of the early diagnosis of GBS. The diagnostic value of motor nerve conduction velocity was higher than that of sensory nerve conduction velocity in the diagnosis of GBS.
