摘 要:目的:探讨Er,Cr:YSGG激光蚀刻对金属托槽与牙釉质粘接强度的影响。方法:选取因正畸拔除的前磨牙80颗,根据不同的牙釉质表面预处理方式将试件分为磷酸酸蚀组和Er,Cr:YSGG激光蚀刻组。根据不同的检测时间点又分为4个亚组(n=10),粘接后30 min组(a组),粘接后24 h组(b组),冷热循环5 000次组(c组),冷热循环10 000次组(d组)。检测各组试件的抗剪切强度并在4倍体式显微镜下统计评估粘接剂残留指数。结果:在磷酸组和Er,Cr:YSGG激光组,粘接后24 h组抗剪切强度均最高(P<0.05),而粘接后30 min组的抗剪切强度值均最低(P<0.05)。粘接后24 h组、冷热循环5 000次组和冷热循环10 000次组,Er,Cr:YSGG激光处理组的抗剪切强度值均高于磷酸组(P<0.05)。在同一检验时间点时,Er,Cr:YSGG激光组的ARI积分与磷酸处理组对比差异并无统计学意义;在两组中,粘接后30 min组ARI积分均最高,随着试验老化时间的增加,其相应的ARI积分也逐渐降低。结论:经Er,Cr:YSGG蚀刻的牙釉质与金属托槽短期粘接效果优于酸蚀处理技术。Objective: The purpose of this study was to evaluate the effect of Er, Cr : YSGG laser-based etch on the bond strength of the metal brackets to enamel, netheds: Eighty human premolars extracted for orthodontic treatment were ob- tained to made into the specimens for shear bond test, and then randomly divided into 2 groups according to the different en- amel surface pre-treatments that were etched by phosphoric acid( group A)and by Er, Cr:YSGG laser( group B). And then, both groups were randomly divided into 4 subgroups (n = 10)on the basis of the different testing times, that were after the bond for 30 min ( subgroup a) , after the bond for 24 h ( subgroup b) , thermal-cycling for 5000 times ( subgroup c ), thermal-cy- cling for 10000 times( subgroup d) ,respectively. The shear bond strength test was used to evaluate the bond strength of the metal brackets to enamel, followed by the counts of the adhesive remnant index (ARI). Results : In the group A, the subgroup Ab was obtained the highest shear bond strength(P 〈0. 05) ,so was the subgroup Bb in the group B(P 〈 0. 05). However,the shear bond strength of the subgroup Aa and Ba was the lowest in the group A and B, respectively. After the bond for 30 min, there was no significant difference between group A and B ( P 〉 0. 05 ), while under the conditions of the other three testing periods, the shear bond strength of group B was significant higher than group A (P 〈 0. 05 ). For all of the four test time points,the ARI of the group A was not significant higher than the group B(P 〉0. 05). Both of group A and B,the ARI were highest in subgroup a, that along with the increase of the aging time, the ARI of the subgroups became lower and lower. Con- clusion:Compared with the phosphoric acid etching, Er, Cr:YSGG laser-based etch was more effective to the bond strength of metal brackets to enamel, especially for the short-term effects.
