三种牙色桩核系统全瓷冠修复后抗折强度研究

摘  要:目的:比较3种牙色桩核系统全瓷冠修复后整体抗折强度。方法:将30颗完整离体上颌中切牙,随机分为3组,A组(遮色钴铬合金桩核组)、B组(氧化锆全瓷桩核组)和C组(纤维桩树脂核组),每组各10颗。所有离体牙均保留1.5 mm牙本质肩领,桩核全瓷冠修复后,运用力学万能实验机测量3组桩核全瓷冠整体抗折强度并观察其折裂模式。结果:3组修复体整体抗折强度依次为,528.35±41.58 N,467.29±37.52 N和372.48±51.34 N,其中C组与A和B组间差异均有统计学意义(P<0.05),A、B两组间差异无统计学意义(P>0.05)。3组最终破损模式表现为:A、B组主要表现为牙根尖1/3处折裂和牙根纵折,破坏多为不可修复性;C组的折裂多发生在牙根颈1/3,多为可修复性。结论:在1.5 mm牙本质肩领时,为防折裂,建议少选纤维桩树脂核;可考虑遮色钴铬合金桩核或氧化锆全瓷桩核。Objective :To compare the overall fracture resistance of three kinds of teeth-color post-core systems with ceramic crows. Methods : A total of 30 intact extracted maxillary central incisors were selected and divided into three groups with 10 incisors for each group. They were the group of color-masking Co-Cr post-cores ( group A) , ZrO2 post-cores ( group B) , and fiber-resin post-cores ( group C) that were all remained 1.5 mm dentine shoulder edge. The fracture resistance of the three groups was measured a universal load-testing machine after ceramic crows repaired and the fracture modes were ob- served. Results : The fracture resistance of the three was 528.35 ± 41.58 N,467. 29± 37. 52 N and 372.48 ± 51.34 N, respectively. There was significant difference in resistance among three groups ( P 〈 0. 05 ). No significant difference was ob- served between group A with B ( P 〉 0. 05 ). The finer fracture modes of the three were that group A and B were fractured or vertical ruptured at 1/3 of the root tip which can' t been repaired, and group C was fractured at the root cervical portion that can be repaired. Conclusion : It' s suggested that less fiber-resin post-cores at the 1.5 mm dentine shoulder edge, the ZrOz post-cores and color-masking Co-Cr post-cores were better.
