LASIK及FS-LASIK术中游离瓣原因与处理

摘　　要：目的回顾分析准分子激光原位角膜磨镶术（LASIK）以及飞秒激光制瓣的FS-LASIK术中游离瓣的原因、预防以及处理方法。方法对2012年3月至2016年3月在厦门眼科中心行LASIK手术7 011例（13 931只眼）以及FS-LASIK 8 611例（17 123只眼），术中出现的11只眼游离瓣的原因和术后恢复情况进行回顾分析。结果术后1 d复查，11只眼游离瓣均复位良好。其中9只眼一次完成手术者，术后3d时取出角膜接触镜，8只眼视力≥0.8，一个月后均达到术前最佳矫正视力，仅1只眼术后第二天视力为0.25，术后1个月达到0.6；另外2只眼改期行T-PRK的患者，术后一个月视力均≥0.8，达到术前最佳矫正视力。术后6个月复查，上述患者视力稳定，且无其他并发症发生。结论LASIK或FS-LASIK术中若出现游离瓣，及时准确的判断和正确的处理后患者仍然可以获得满意的治疗效果。ObjectiveTo evaluate the causes and treatments of flap dislocation during laser-in-situ-keratomileusis（LASIK） and femtosecond laser-assisted laser in situ keratomileusis（FS-LASIK）.MethodsEleven eyes of flap dislocation during LASIK（7011 cases, 13931 eyes） and FS-LASIK（8611 cases, 17123 eyes） were enrolled.ResultsOn the 2nd day, all the corneal flaps were better recovered, visual acuity in 8 eyes were 〉0.8, only 1 eye was 0.25. Visual acuity in 1 month of the 8 eyes all reached the best corrective vision and kept stability for 6 months except 1 eye only reached 0.6 （lower than its best corrective vision 0.7）. The other 2 eyes underwent Trans-PRK after 3 months, and the visual acuity was above 0.8 1m after surgery.ConclusionsCorneal flap dislocation also can be managed effectively and have a good result after treatment.
