摘 要:目的:通过比较不同病理分期胃癌患者的同型半胱氨酸(Hcy)水平,探讨血清Hcy与胃癌及病理分期的相关性。方法:选取2014年3月—2015年6月于山东大学齐鲁医院住院的胃癌患者138例,根据美国癌症联合会第七版胃癌分期指南分为4组:Ⅰ期(n=34)、Ⅱ期(n=33)、Ⅲ期(n=58)、Ⅳ期(n=13),并以同时期入院的良性胃疾病患者为对照(n=90),比较良性胃疾病与胃癌及各分期间Hcy及肿瘤标志物水平差异,分析Hcy与胃癌不同病理分期的相关性。采用单循环酶法测定血清Hcy水平。结果:良性胃疾病患者血清Hcy水平(12.31±3.22)μmol/L,胃癌患者(16.19±4.84)μmol/L,胃癌患者血清Hcy水平显著升高(P<0.05)。血清Hcy水平随胃癌临床分期增高逐渐升高,其中I期(13.94±4.07)μmol/L,Ⅱ期(15.49±4.09)μmol/L,Ⅲ期(17.10±4.79)μmol/L,Ⅳ期(19.81±5.77)μmol/L,各分期差异有统计学意义(P<0.05)。相关性分析证实,血清Hcy与病理分期呈正相关(r=0.503,P<0.001)。在校正了其他胃癌危险因素后,Logistic回归分析显示,血清Hcy仍是胃癌发生的独立危险因素(OR=1.208,P=0.003)。结论:血清Hcy水平与胃癌及临床病理分期密切相关,伴随临床分期进展Hcy水平明显升高,对判定胃癌病情发展有重要价值。Objective: To study the correlation between serum homocysteine (Hcy) and the staging of gastric cancer, by comparing the concentrations of patients with gastric cancer at different pathological staging. Methods: 90 patients with benign gastric diseases and 138 patients with gas-tric cancer were selected and admitted by Shandong University Qilu Hospital during the date from Mar 2014 to Jun 2015. The patients with gastric cancer were divided into 4 groups, according to the 7th AJCC Cancer Staging. To compare the difference of the concentration levels of Hcy and Tumor marks in different groups and analyze the relationship between benign disease and gastric cancer, and analyze it correlation with different pathological stagings of gastric cancer. Enzymatic cycling as- say was used for detecting serum Hcy. Results: The serum Hcy concentration level in benign dis- ease was (12.31 ±3.22) μ mol/L, and significantly lower than cancer group(16.19±4.84) μmol/L, the difference was statistically significant (P〈O.05). The serum Hcy concentration levels increased grad- ually from staging I to staging IV, and the concentration level in staging I was (13.94±4.07) μ mol/L, in staging II was (15.49±4.09) μmol/L, in staging Ⅲ was (17.10±4.79) μmol/L, in staginglV was (19.81±5.77) μmol/L, the differences among the four groups were statistically significant(P〈O.05). Spearman Rank Correlation analysis confirmed that, the Hcy concentration level was positively relat- ed with pathological staging(r=0.503, P〈 0.001). Logistic regression analysis showed that, the serum Hcy concentration level is significantly correlated with gastric cancer, after the adjustment of other risk factors (OR=1.208, P=O.O03). Conclusions: The serum Hcy concentration level is closely cor- related with gastric cancer staging, and increase significantly with the cancer staging (from staging I to staging IV), so it can be used to evaluate the severity of gastric cancer.
