甲状腺球蛋白检测联合颈部超声对甲状腺癌的诊断分析

摘　　要：目的观察评价甲状腺球蛋白（Thyroglobulin，TG）检测联合颈部超声对甲状腺癌（Differentiated thyroid carcinoma，DTC）疗效的应用价值。方法选择2015年1月至2016年9月期间回顾性分析东莞市人民医院收治及本院诊治的甲状腺癌患者48例，所有患者检测血清TG水平，同时实施颈部超声检查，并进行全身SPECT扫捕（WBS）。结果TG对DTC复发或转移灶诊断的灵敏度为76．87％、特异性为66．66％和准确性为75．00％；超声诊断的DTC治疗后淋巴结转移的灵敏度为79．00％、特异性为58．00％、准确性为75．00％；TG联合超声（两者均阳性或均阴性时）诊断的灵敏度为96．77％、特异性为71．39％、准确性为92．09％。结论TG检测结合颈部超声可以明显提高对DTC治疗后复发或转移灶诊断的灵敏度、特异性和准确性。血清TG水平在DTC术前诊断及术后监测中均起显著作用。Objective To evaluate the value of thyroglobulin （TG） and thyroid ultrasonography in the diagnosis of thyroid carcinoma （DTC）. Mehtods 48 patients with thyroid cancer treated at Dongguan People＇s Hospital from January, 2015 to September, 2016 were enrolled in this study. All patients were examined for serum TG levels. Neck ultrasonography and systemic SPECT were performed. （WBS）. Results The diagnostic sensitivity, specificity, and accuracy of TG were 76.87%, 66.66% and 75.00%, those of ultrasonography were 79.00%, 58.00%, and 75.00%, and those of the combination of these two were 96.77%, 71.39%, and 92.09%, respectively. Conclusions Thyroglobulin （TG） detection combined with ultrasound for the recurrence or metastases of can significantly improve the sensitivity, specificity, and accuracy. Serum TG level plays a significant role in the preoperative diagnosis of and postoperative monitoring DTC.
