2012-2016年北京市昌平区感染性腹泻病原菌检测结果分析

摘　　要：目的分析北京市昌平区2012-2016年感染性腹泻病原菌构成、流行特征及主要致病菌血清型分布,为本地区感染性腹泻防控提供参考依据。方法收集2012-2016年北京市昌平区2家哨点医院肠道门诊腹泻患者的粪便标本进行志贺菌、沙门菌、致泻性大肠埃希菌、副溶血性弧菌等肠道致病菌的检验。结果 1 948份标本中,395份标本检出病原菌,总阳性率为20.28%。副溶血性弧菌、沙门菌、致泻性大肠埃希菌、志贺菌的阳性检出率分别为6.31%、6.26%、5.65%、3.39%。8月份检出率最高。30~39岁年龄组病原菌检出率最高为26.03%(101/388)(x^2=23.294,P=0.001)。副溶血性弧菌以O3:K6血清型为主,致泻性大肠埃希菌以EPEC为主,志贺菌以宋内志贺菌为主,沙门菌以肠炎沙门菌血清型为主。结论北京市昌平区细菌性腹泻病原菌主要以副溶血性弧菌为主,其次为沙门菌、致泻性大肠埃希菌、志贺菌,但不同年份细菌谱存在差异。Objective To analyze the constitution, epidemiological features and serotype distribution of infectious diarrhea pathogenic bacteria in Changping district of Beijing from 2012 to 2016, and to provide reference basis for infectious diarrhea prevention and control. Methods Stool samples were collected from diarrhea patients in 2 sentinel hospitals from 2012 to 2016 to conduct the detection of Shigella spp. , Salmonella spp. , Diarrhea escherichia coli and Vibrio parahaemolyticus. Results Pathogen bacteria was detected in 395 specimens, and the total positive rate was 20. 28%. The positive rate of Vibrio parahaemolyticus,Salmonella spp. , Diarrhea escherichia coli and shigella spp. were 6.31% , 6.26% , 5.65% and 3.39% , respectively. The highest detection rate appeared in August. The pathogen detection rate of the 30 - 39 year - old age group was as high as 26. 03% （101/388） , which was the hightest （χ^2= 23. 294, P = 0. 001 ）. The major serotype of Vibrio parahaemolyticus, Diarrhea escherichia coli, Shigella spp. , Salmonella spp. were O3： K6, EPEC, Shigella Sonnei, Salmonella Enteritidis, respectively. Conclusion Vibrio parahaemolyticus is the main pathogens of diarrhea, followed by Sahnonella spp. , Diarrhea escherichia coli, Shigella spp.. The bacteria distribution is different in different years.
