6%羟乙基淀粉130/0.4和呋噻米在急性胰腺炎液体复苏中的联合应用

摘　　要：目的观察静脉输注6%羟乙基淀粉130/0.4和联合静脉推注呋噻米对急性胰腺炎转归的影响。方法 120例急性胰腺炎患者随机分为观察组与对照组各60例。对照组行常规治疗,观察组在常规治疗的基础上给予6%羟乙基淀粉130/0.4联合注呋噻米治疗。记录两组患者尿量、并发症、恢复指标和平均住院日。结果观察组尿量多于对照组;观察组红细胞压积的下降速度明显快于对照组;各种并发症发生率均低于对照组;观察组恢复指标时间和平均住院日均小于对照组,差异均有统计学意义(P<0.05)。结论静脉输注羟乙基淀粉和静脉推注呋噻米可降低胰腺炎并发症的发生,加速患者康复,降低平均住院床日,改善急性胰腺炎的转归。Objective To observe the effect of combination of intravenous infusion of 6% hydroxyethyl starch 130/0. 4 and in- travenous injection of furosemide （Lasix） on the outcome of patients with acute pancreatitis. Methods One hundred and twenty pa- tients with acute pancreatitis were randomly divided into observation or control groups ,60 in each group. The control group received rou- tine treatment while the observation group was given the therapy of 6% hydroxyethyl starch 130/0.4 combined with injection of furose- mide based on the conventional treatment. The urinary volume, complications, recovery indexes and average days of hospital stay were compared between the two groups. Results The urinary volume, the decrease rate of hematoerit, the incidence of complications, the time of recovery indexes and average days of hospital stay in the observation group was better than those in the control group （ all P 〈 0. 05 ）. Conclusion The combination of intravenous infusion of 6% hydroxyethyl starch 130/0. 4 and intravenous injection of furose- mide can decrease the occurrence of complications, accelerate the patients recover, reduce the average hospital stay and improve the out- come of the disease.
