腹腔镜手术与开腹手术治疗早期子宫内膜癌对比分析

摘　　要：目的对比腹腔镜手术与开腹手术治疗早期子宫内膜癌的优劣,为早期子宫内膜癌治疗方案的选择提供依据。方法选取2012年2月至2015年1月就诊行手术治疗的随访12个月以上的Ⅰ期(FIGO分期)子宫内膜癌患者69例。依据手术方法的不同,将行腹腔镜手术治疗者设为腹腔镜组(36例),行开腹手术治疗者设为开腹组(33例)。比较记录患者的手术结果、手术时间、术中出血量、术中清扫淋巴结数目、术后行止痛治疗、术后肛门首次排气时间、引流管拔除时间、术后住院时间、住院总花费、切口满意度评分(Kiyak满意度标准)、术中及术后并发症、复发、死亡等情况。结果 (1)腹腔镜组1例因粘连过重、术中出血难以控制而中转开腹,69例患者手术均成功,未发生复发及死亡。(2)两组术中出血量、术中清扫淋巴结数目、引流管拔除时间接近(P>0.05);与开腹组相比,腹腔镜组手术时间长、住院总花费多、术后肛门首次排气时间早、住院时间短、切口满意度高、术后疼痛轻(P<0.05);腹腔镜组术中、术后并发症发生率分别为8.57%、20.00%,与开腹组的9.09%、24.24%接近(P>0.05)。结论腹腔镜手术与开腹手术均是早期子宫内膜癌行之有效的治疗方式,开腹手术用时少、花费少,腹腔镜手术切口满意度高、疼痛轻、恢复快,治疗中需根据具体情况加以选择。Objective To compare the advantages and disadvantages of laparoscopic surgery and open surgery in the treatment of early endometrial cancer, and to provide a theoretical basis for the selection of early endometrial cancer treatment program.Methods From February 2012 to January 2015, 69 cases of endometrial cancer stage I （FIGO stage） patients with surgical treatment and more than 12 months follow-up were selected in this study, according to different surgical methods, the patients were divided into laparoscopic group （36 cases） and open surgery group （33 cases）.The results of operation, operation time, intraoperative blood loss, number of lymph nodes dissection after operation, analgesic therapy, postoperative anal exhaust time, extubation time, postoperative hospitalization time, hospitalization costs and incision satisfaction scores （Kiyak satisfaction）, complications, recurrence and death situation during and after the operation were compared.Results 1 case in laparoscopic group converted to open surgery group because of excessive adhesion and difficult control of intraoperative bleeding, 69 cases were operated successfully.The bleeding volume, postoperative cleaning lymph node number, drainage tube removal time in the two groups had no significant difference （P〉0.05）.Compared with open surgery, laparoscopic surgery group had the characteristics of a long surgery time, more hospitalization cost, earlier first postoperative anal exhaust time, shorter hospitalization time, higher satisfaction of incision and slighter postoperative pain （P〈0.05）;Complication rate of the laparoscopic group intraoperative and postoperative was 8.57%, 20.00% respectively, and had no significant difference with that of the open group, 9.09%, 24.24% （P〉0.05）.Conclusion The laparoscopic surgery and open surgery are effective early endometrial cancer treatment methods.Open surgery has the advantage of less surgery time and cost, laparoscopy has the advantage of higher satisfaction of incision, slighter
