康复医院-社区-家庭无缝对接护理干预模式对精神分裂症患者用药依从性及康复效能的影响

摘　　要：目的:探讨康复医院-社区-家庭无缝对接护理干预模式对精神分裂症患者用药依从性及康复效能的影响结果。方法:选取我院2015年10月至2016年10月住院治疗的60例男性精神分裂症患者,按照入院日期随机均分为干预组(n=30)和对照组(n=30),对照组接受常规护理和用药指导及出院随访,干预组给予系列住院期间优质护理干预、个案管理、用药指导、康复训练、出院后联合社区卫生服务专职人员共同跟踪指导和定期入户随访等无缝对接护理干预模式。于两组患者出院后6个月比较用药依从性和康复效能。结果:干预组患者用药依从性评价总依从率明显高于对照组,干预组出院后患者康复效能PSP指数评价同期平均得分均明显高于对照组,差异有统计学意义(P<0.05)。结论:康复医院-社区-家庭无缝对接护理干预模式,能有效提高精神分裂症患者的用药依从性,显著改善和促进患者的康复效能,提高治疗效果,降低疾病的复发率,值得临床推广应用。ObjectiveTo explore the effect of rehabilitation model of hospital rehabilitation on the compliance and rehabilitation of patients with schizophrenia. Methods ： Fifty patients with male schizophrenia who were hospitalized from October 2015 to October 2016 were randomly divided into intervention group （n=30） and control group （n=30） according to the date of admis-sion The control group received routine Nursing and medication guidance and discharge follow-up, the intervention group to give a series of hospital care during the high-quality care intervention, case management,medication guidance,rehabilitation training, post-discharge community health services full-time staff co -tracking and regular follow-up follow-up and other seam-less care intervention mode The compliance and rehabilitation efficacy were compared between the two groups at 6 months after discharge. Results： The overall compliance rate of the patients in the intervention group was significantly higher than that in the control group （P 〈 0. 05）,and the difference was statistically significant （P 〈0. 05）. Conclusion： Rehabilitation hospital- community-family seamless docking nursing intervention model, can effectively improve the compliance of patients with schizo-phrenia, significantly improve and promote the rehabilitation of patients, improve the treatment effect, reduce the recurrence rate of the disease, worthy of clinical application
