Luminal A型和Luminal B型乳腺癌患者的临床特征和预后

摘　　要：目的:研究Luminal A型和Luminal B型乳腺癌患者的临床特征和预后情况。方法:选择2014年1月至2015年12月进行诊治的乳腺癌患者170例,采用免疫组织化学检测方法,依据孕激素受体(progesterone receptor,PR)、雌激素受体(estrogen receptor,ER)、Ki67以及人表皮生长因子受体-2(human epidermal growth factor receptor-2,HER-2)等指标的表达情况进行分子分型,分出Luminal A型乳腺癌患者83例,Luminal B型乳腺癌患者87例,观察Luminal A型和Luminal B型乳腺癌患者的临床特征和预后情况。结果:与Luminal A型乳腺癌患者相比,Luminal B型乳腺癌患者的年龄较小(≤50岁)(P<0.05),腋窝淋巴结转移率较高(P<0.05),肿瘤直径较大(P<0.05),脉管易受到侵犯(P<0.05);而Luminal A型和Luminal B型乳腺癌患者在家族史、体质量指数、肿瘤位置、组织病理等方面差异无统计学意义(P>0.05);经过3年的随访,Luminal A型乳腺癌患者有14例(16.86%)发生复发转移,7例(8.43%)发生死亡,3年生存率为91.57%;Luminal B型乳腺癌患者有11例(12.64%)发生复发转移,15例(17.24%)发生死亡,3年生存率为82.76%;两者相比差异有统计学意义(P<0.05)。结论:与Luminal A型乳腺癌患者相比,Luminal B型乳腺癌患者具有更严重的临床表现,且预后更差。Objective ： To study the clinical features and prognosis of breast cancer patients with type of Luminal A and Luminal B. Methods： 170 cases of patients with breast cancer from January 2014 to December 2015 were selected. Immunohistochemistry method was adopted to take mo- lecular typing according to the expression of progesterone receptor （PR）, estrogen receptor （ER）, Ki67, human epidermal growth factor receptor - 2 （ HER - 2 ） and other indicators. 83 cases with type of Luminal A and 87 cases with type of Luminal B were selected, and the clinical features and prognosis in the two types were compared. Results： Compared with those cases with type of Luminal A, the breast cancer patients with type of Luminal B were significantly younger （ ~〈50 years old） （ P 〈 0.05 ） , their axillary lymph node metastasis rate was higher （ P 〈 0.05 ）, the tumor diameter was bigger （ P 〈 0.05 ）, and the pulse tube was easier to be invaded （ P 〈 0.05 ）. But there were no significant differences of their fam- ily history, body mass index, tumor location and tissue pathology in those patients with type of Luminal A and Luminal B （ P 〉 0.05 ）. After 3 years of follow - up, there were 14 cases （ 16.86% ） of recurrence and metastasis and 7 cases （ 8.43 % ） died in patients with type of Luminal A, and the 3 - year survival rate was 91.57 %. However, after 3 years of follow - up in patients with type of Luminal B, there were 11 cases （ 12.64 % ） of recurrence and metastasis and 15 cases （ 17.24 % ） died, and the 3 - year survival rate was 82.76 %. There was significant differences be- tween the two types （ P 〈 0.05 ）. Conclusion ： Compared with those cases with type of Luminal A, breast cancer patients with type of Luminal B have the worse clinical pathological characteristics and prognosis.
