同期经尿道手术治疗浅表膀胱肿瘤合并前列腺增生的临床观察

摘　　要：目的探讨同期经尿道手术治疗浅表膀胱肿瘤合并前列腺增生的疗效。方法选取我院2011年5月至2015年5月收治的60例浅表膀胱肿瘤合并前列腺增生患者,随机分为两组各30例。对照组仅给予TURBT治疗,观察组在对照组基础上同期行TURP治疗。观察两组患者的疗效。结果观察组的手术时间、术后导尿管留置时间、术中出血量、住院时间均显著优于对照组(P均<0.05)。观察组的IPSS、RUV均显著低于对照组,Q_(max)显著高于对照组(P均<0.05)。术后1年内,对照组的肿瘤复发率和尿道狭窄发生率均显著低于观察组(P<0.05)。结论 TURBT同期行TURP治疗浅表膀胱肿瘤合并前列腺增生患者,虽然手术创伤大,术后恢复慢,但其疗效显著,术后复发率低,具有临床推广价值。Objective To explore the curative effect of homeochronous transurethral surgery in the treatment of superficial bladder tumor complicated with prostatic hyperplasia. Methods 60 cases of patients with superficial bladder tumor complicated with prostatic hyperplasia admitted to our hospital from May 2011 to May 2015 were selected and randomly divided into two groups, with 30 cases in each group. The control group only received TURBT, while the observation group received homeochronous TURP on the basis of control group. The curative effect of patients in two groups was observed. Results The operation time, postoperative retention time of catheter, intraoperative blood loss and hospitalization time of control group were significantly better than those of observation group （all P〈0.05）. The IPSS and PVR of observation group were significantly lower than those of control group, and the Qmax was significantly higher than that of control group （all P 〈0.05）. A year after surgery, the tumor recurrence rate and incidence of urethral stricture of observation group were significantly lower than those of control group （P〈0.05）. Conclusions Although homeochronous TURP and TURB.T in the treatment of patients with ssuperficial bladder tumor complicated with prostatic hyperplasia have severe surgical trauma and slow postoperative recovery, but the curative effect is significant and the postoperative recurrence rate is low. They have the value of clinical promotion.
