摘 要:目的:探讨中药黄龙汤对脓毒症大鼠免疫功能的保护作用及机制。方法:将55只Wistar大鼠按随机数字表法分为假手术组(10只)、模型组(15只)、西药治疗组(15只)和中西药联合治疗组(15只)。采用盲肠结扎穿孔术(CLP)复制脓毒症模型,假手术组只手术,不行CLP。假手术组和模型组灌胃2 m L生理盐水;西药对照组腹腔注射美罗培南每次50 mg;中西药联合治疗组在西药治疗基础上灌胃中药黄龙汤[生大黄(后下)15 g,芒硝10 g,枳实15 g,厚朴10 g,人参10 g,当归20 g,黄芩15 g,甘草10 g,大枣12 g,煎浓成150 m L]每次2 m L(含生药0.68 g/m L),4组均每日2次。给药4 d后处死大鼠,腹主动脉取血。运用流式细胞仪检测T淋巴细胞亚群CD4、CD8的变化。结果:本研究结果显示,西药组、中药+西药组回肠黏膜淋巴细胞中CD_4^+、CD_8^+T细胞及CD_4^+T/CD_8^+T值均较模型对照组显著上升(P<0.05)。中药+西药组CD_4^+T细胞较西药组明显升高(P<0.05)。结论:中药汤剂黄龙汤在脓毒症大鼠中具有提高CD_4^+T细胞计数,对抗脓毒症免疫抑制,提高脓毒症模型大鼠生存率的作用。Objective: To investigate the protective effect and mechanism of traditional Chinese medicine Huanglong Decoction on immune function in rats with sepsis. Methods: 55 Wistar rats were randomly divided into sham operation group ( 10 rats ), model group ( 15 rats ), Western medicine group ( 15 rats ) and combined treatment group in Western medicine and Huanglong Decoction ( 15 rats ). Sham operation group only open abdominal cavity and suture. Other groups replication model of sepsis by cecal ligation and puncture ( CLP ). Sham operation group and model group were fed by 2 mL saline, bid. The Western medicine treatment group rats were treated by meropenem 50 mg, intraperitoneal injection, bid; rats in combined treatment group treated by meropenem 50 mg, intraperitoneal injection, bid, while Huanglong Decoction ( Rheum palmatnm 15 g, Mirabilite 10 g, Citrus aurantium 15 g, Magnolia officinalis 10 g, Panax ginseng 10 g, Angelica sinensis 20 g, Seutellaria baicalensis 15 g, Glycyrrhiza uralensis 10 g, Ziziphus jujuba 12 g, decoct into 150 mL ), also given 2 mL oral feeding, bid. Rats were killed after administration for 4 d, CD4^+ T and CD8^+ T in blood which taken from abdominal aorta were detected by flow cytometry. Result: CO4^+ T, CO8^+ T and CD4^+ T/CD8^+ T in Western medicine group and combined treatment group were expressed higher than model group ( P〈0.05 ). CD4^+ T in combined treatment group got a higher expressions than Western medicine group ( P〈0.01 ). Conclusion : Huanglong Decoction on raise the level of CD4^+ T, to fight the immunosuppression of sepsis and improve the survival rate of septic model rats.
