白头翁汤对溃疡性结肠炎患者Th17/Treg细胞失衡调节作用

摘　　要：目的:观察白头翁汤对溃疡性结肠炎患者Th17/Treg细胞失衡的调节作用,以探讨其治疗UC的可能机制。方法:将40例UC患者随机分为中药治疗组和西药对照组,分别给予白头翁汤内服外灌及口服美沙拉嗪缓释颗粒。采用流式细胞术检测UC患者外周血中Th17细胞、Treg细胞占总CD4+T细胞百分比并计算Th17/Treg比率,采用Western blot检测结直肠组织RORγt和Foxp3蛋白的表达。结果:白头翁汤治疗后Th17细胞比例及Th17/Treg较治疗前明显减少(P<0.01),较西药对照组减少(P<0.05);Treg细胞比例较治疗前明显增加(P<0.01),较西药对照组增加(P<0.01)。白头翁汤治疗后能下调RORγt表达(P<0.01),上调Foxp3表达(P<0.01)。结论:白头翁汤对溃疡性结肠炎患者有明显的治疗作用,其作用机制可能与提高患者Treg细胞比例、降低Th17细胞比例,进而恢复机体Th17/Treg平衡,抑制肠道炎症有关。Objective：To observe possible mechanism of Baitouweng Decoction＇s therapeutic effect through regulating the balance of Th17/Treg in patients with ulcerative colitis. Methods：Totally 40 patients with UC were randomly assigned to the treatment group and the control group. Besides,another 20 healthy volunteers were recruited as the healthy control group. Patients in treatment group were treated with Baitouweng Decoction（oral and enema treatment）,and those in control group took Mesalazine. The proportion of Th17 and Treg cells in peripheral blood CD4-＋ T cells and the ratio of Th17/Treg were detected by flow cytometry. The expression of Foxp3 and RORγt was determined by Western blot analysis. Results：The proportion of Th17 cells was significantly decreased（P〈0.01）,and the proportion of Treg cells was significantly increased after treated with Baitouweng Decoction（P〈0.01）. The ratio of Th17/Treg was significantly decreased after treated with Baitouweng Decoction（P〈0.01）. The expression of Foxp3 was increased（P〈0.01）and the expression of RORγt was decreased（P〈0.01）after treated with BaitouwengDecoction. Conclusion：Baitouweng Decoction can effectively inhibit inflammation,and the therapeutic mechanism may be related with the regulation of Th17/Treg balance through increasing the proportion of Treg cells and lowering the proportionof of Th17 cells in patients with UC.
