摘 要:目的:探讨MUC1单抗4G10在肺腺癌中的染色情况及临床意义,为开发新的肺腺癌肿瘤免疫治疗方法提供理论依据。方法:以4G10作为一抗,C595作为对照,应用免疫组织化学方法检测342例肺腺癌和345例鳞癌中MUC1蛋白的表达情况。结果:肺腺癌组织中4G10染色多呈强阳性、弥漫性(2+~3+),阳性率为90.64%(310/342),明显高于C595的80.41%(275/342),而肺鳞癌组织中的阳性率为11.59%(40/345)(P<0.000 1),则低于C595的13.62%(47/345),且染色多呈阴性或弱阳性(0~1+)(P<0.000 1)。342例肺腺癌中4G10的阳性率及染色强度与患者性别、年龄和有无吸烟史之间均不相关(P1=0.073 9,P2=0.160 3;P1=0.935 8,P2=0.936 6;P1=0.369 5,P2=0.618 4),但随着肿瘤恶性程度的增加、临床分期的递进及淋巴结的转移,4G10阳性率与染色强度增加,均分别呈现正相关趋势(P1=0.000 9,P2=0.000 3;P1=0.052 7,P2=0.039;P1=0.106 7,P2=0.037 4)。结论:MUC1的异常表达与肺腺癌发生发展、侵袭、转移密切相关,可作为肺腺癌临床诊断的参考指标,对肺腺癌的恶性程度及预后判断有重要参考价值,并在肺腺癌的肿瘤免疫治疗中有望成为新的潜在靶点,发挥重要作用。Objective:In this study,the clinicopathological significance and staining of MUC1(4G10) protein in lung adenocarcinoma were discussed,which provide a theoretical basis for the development of new tumor immunotherapy in lung adenocarcinoma.Methods:Using 4G10 and C595 as primary antibodies respectively,the staining of 4G10 protein in 342 specimens of lung adenocarcinoma and 345 specimens of squamous cell carcinoma were detected by using immunohistochemical technique.Results:The positive rates of 4G10 staining was 90.64%(310/342) in lung adenocarcinoma,which was significantly higher than that of C595.Besides,the positive rates of 4G10 staining was 11.59%(40/345) in squamous cell carcinoma (P&lt;0.000 1),which was lower than that of C595.In addition,the positive rates and staining level of 4G10 was significantly associated with pathological grade,TNM stage and lymphatic metastasis in 342 specimens of lung adenocarcinoma(P1=0.000 9,P2=0.000 3.P1=0.052 7,P2=0.039.P1=0.106 7,P2 =0.037 4),but were irrelevant to gender,age and smoking history.Conclusion:Abnormal expression of MUC1 protein is closely related to lung adenocarcinoma occurance,development,invasion and metastasis,suggesting that it may be an important biological markers for clinical diagnosis and a great reference in estimating the malignancy and prognosis of lung adenocarcinoma.MUC1 could serve as an attractive target and play an important role in tumor immunotherapy in lung adenocarcinoma.
