酒精依赖患者全脑灰质变化的影像学Meta分析

摘　　要：目的:利用激活似然估计(ALE,Activation Likelihood Estimate)的Meta分析方法对酒精依赖患者的脑灰质变化情况进行分析、归纳和讨论。方法:在医学数据库中检索从2000年1月-2014年11月运用基于体素的形态学分析方法(Voxel-based Morphometry,VBM)对酒精依赖患者脑灰质体积萎缩进行研究的文献,然后应用基于ALE的Meta分析方法进行归纳。结果:入组了281个酒精依赖患者和344有控制力的健康对照者的8篇研究,研究发现酒精依赖患者的左、右大脑扣带回前部(Brodmann 32区),左侧丘脑和左侧岛叶后部(Brodmann 13区)的灰质体积与健康对照组比较显著减少。结论:本研究结果可以帮助我们从脑结构的角度加深对酒精依赖患者病理、生理状况的理解。Objective： To investigate the concurrence across Voxel-based Morphometry（ VBM） studies to help clarify the structural abnormalities in alcohol dependence.Methods： A systematic search from January,2000 to November,2014 was performed to identify VBM studies that compared alcohol dependent patients and healthy controls. A quantitative meta-analysis of whole-brain VBM studies to estimate regional grey matter abnormalities in alcohol dependent patients was performed using the Activation Likelihood Estimate（ ALE） method.Results： Eight studies consisting of 281 alcohol dependent patients and 344 healthy controls were included in the present meta-analyses. Regional grey matter atrophy in alcohol dependent patients was found in the two sides dorsal anterior cingulate cortex（ Brodmann 32 area）,left thalamus and left posterior of the insular cortex（ Brodmann 13 area）.Conclusion： Our study demonstrated a specific pattern of grey matter abnormalities and provided further insights into understanding the underlying nature of alcohol dependence.
