不同RBH上颌后牙缺失患者行OSFE不植骨同期种植术的疗效差异性比较

摘　　要：【目的】比较分析不同剩余牙槽骨高度(residual bone height,RBH)上颌后牙缺失患者行上颌窦底内提升(osteotome sinus floor elevation,OSFE)不植骨同期种植术的疗效差异。【方法】120例上颌后牙缺失患者(RBH值4~8 mm)均行OSFE不植骨同期种植术,种植体共163颗,其中4 mm≤RBH<5 mm种植53颗(设为实验组),5 mm≤RBH≤8 mm种植110颗(设为对照组),比较两组种植体成功率、种植体周边组织状况及患者主观满意度(视觉模拟评分)。【结果】种植体总成功率为99.39%,实验组种植体成功率98.11%,对照组种植体成功率100.00%,两组比较差异无统计学意义(P>0.05);实验组患者术后6个月新骨获得量显著多于对照组患者(P<0.05),而术后12个月垂直骨丧失及视觉模拟评分两组比较差异无统计学意义(P>0.05)。【结论】4 mm≤RBH<5 mm与5 mm≤RBH≤8 mm上颌后牙缺失患者行OSFE不植骨同期种植术疗效相当且满意。【Objective】To compare and analyze the therapeutic efficacy of osteotome sinus floor elevation（OSFE） with synchronous implantation and without bone grafting in the treatment for the loss of maxillary posterior teeth with different residual bone height（RBH）.【Methods】A total of 120 cases with the loss of maxillary posterior teeth（RBH 4 ~ 8mm） were treated with OSFE with synchronous implantation and without bone grafting. There were a total of 163 implants, including 53 implants in the experiment group（4 mm≤RBH5mm） and 110 implants in the control group（5 mm≤RBH≤8mm）. The success rate of implants, the surrounding tissue condition of implants and subjective satisfaction of patients（VAS score） were compared between the two groups.【Results】The total success rate of implants was 99.39%. The success rate in the experiment group was 98.11%, while it was 100.00% in the control group. There was no significant difference between the two groups（P〉0.05）. The new bone volume in the experiment group was significantly more than that in the control group 6 months after the operation（P〈0.05）. There was no difference in the vertical bone loss and VAS score between the two groups 12 months after the operation（P〉0.05）.【Conclusion】The therapeutic efficacy of OSFE with synchronous implantation and without bone grafting is satisfactory and similar in the treatment for the loss of maxillary posterior teeth with 4mm≤RBH5mm and 5mm≤RBH≤8mm.
