弥漫大B细胞淋巴瘤组织中c-Myc,Bcl-2,Ki-67的表达及临床意义

摘　　要：目的探究弥漫大B细胞淋巴瘤(DLBCL)患者的c-Myc,Bcl-2,Ki-67在组织中的表达及其临床意义。方法选取潍坊市人民医院2013年2月~2016年2月确诊的80例弥漫性大B细胞淋巴瘤患者的淋巴瘤组织纳入试验组,另选取本院同期80例健康志愿者的淋巴组织纳入对照组。采用免疫组织化学法检测DLBCL患者淋巴组织中c-Myc,Bcl-2,Ki-67蛋白表达情况,统计分析其与DLBCL患者临床特征之间的关系。结果试验组患者淋巴组织中c-Myc,Bcl-2,Ki-67蛋白的阳性表达率分别为63.75%,72.50%,58.75%,对照组分别为37.50%,41.25%,28.75%,两组比较差异显著(P<0.05);c-Myc,Bcl-2,Ki-67在不同性别、年龄及不同临床分期的DLBCL患者中阳性表达率无显著差异(P>0.05)。国际标准化预后指数(IPI)评分3~5分、结外受侵、肿瘤未完全缓解的DLBCL中,c-Myc,Bcl-2蛋白的阳性表达显著高于IPI评分0~2分、非结外受侵、肿瘤完全缓解的患者(P<0.05)。Ki-67的表达在IPI评分、Hans免疫分型、临床疗效、乳酸脱氢酶(LDH)水平中存在显著差异(P<0.05)。结论 DLBCL患者淋巴组织中c-Myc,Bcl-2,Ki-67蛋白的表达水平较高,且常见于肿瘤未完全缓解及IPI评分较高的患者。Objective To explore the expression and clinical effect of c-Myc,Bcl-2,Ki-67 protein in diffuse large B-cell lymphoma（ DLBCL）. Methods Eighty cases of DLBCL patients＇ lymphoma tissue were selected as experimental group,80 cases of healthy persons＇ normal lymphoid tissue were selected as control group. The protein expression levels of c-Myc,Bcl-2,Ki-67 were detected by immunohistochemistry. The correlation of protein expression levels of c-Myc,Bcl-2,Ki-67 among DLBCL patients,the relationship among protein expression levels of c-Myc,Bcl-2,Ki-67 and the clinicopathologica characteristics of DLBCL patients were analyzed. Results The positive expression rate of c-Myc,Bcl-2,Ki-67 protein in the lymph tissue of experimental group patients were 63. 75%,72. 50% and 58. 75%,which were significantly higher than those of control group（ 37. 50%,41. 25%,28. 75%）（ P〈0. 05）. There were no significant differences in the expression levels of c-Myc,Bcl-2,Ki-67 protein in DLBCL patients with different genders,ages and clinical stages（ P〉0. 05）. Positive expression rate of c-Myc and Bcl-2 protein in patients with international prognostic index（ IPI） 3 - 5 score,external invasion and tumor complete remission tumor were significantly higher than those in patients with IPI score 0 - 2,the non-nodal invasion,tumor incomplete remission. The positive expression of Ki-67 in the IPI score,Hans immune classification,clinical curative effect,lactate dehydrogenase（ LDH） level had significant difference. Conclusion Expression levels of c-Myc,Bcl-2,Ki-67 protein are higher in DLBCL patients,and are commonly seen in patients with high IPI score andtumor incomplete remission.
