摘　　要：目的探讨肝内胆管囊腺癌(IBC)临床特征及影响预后的相关因素。方法采用系统性回顾法分析我院2012年1月-2014年5月收治的64例IBC患者的临床资料,并与同期收治的64例肝内胆管囊腺瘤(IBCA)患者临床资料比较,观察比较2组临床特征及IBC组预后情况,并分析归纳影响IBC患者预后的相关因素。结果 (1)2组女性均明显多于男性,但IBC患者男性比例稍高,且IBC患者平均发病年龄明显大于IBCA患者,前者临床症状时间大于后者(P<0.05),2组患者均以腹痛为主要临床表现症状,都有血清CA19-9和CEA不同程度的升高;(2)所有IBC患者中位随访时间为12.6个月,总中位PFS为5.8个月;(3)性别、肿瘤类型、手术方式及肿瘤转移为影响IBC患者预后的独立危险因素存在。结论性别(男)、肿瘤类型(浸润型)、手术方式(局部切除)及肿瘤转移(是)为影响IBC患者预后的独立危险因素存在。Objective To investigate the clinical characteristics and prognostic factors of intrahepatic biliary cystadenocar- cinoma （IBC）. Methods The clinical data of 64 cases of IBC patients admitted to our hospital between January 2012 and May 2014 was analyzed retrospectively and compared with that of another 64 patients with intrahepatic biliary cystadenoma （IBCA）. The clinical characteristics of patients in the two groups were observed and compared before the prognosis of Group IBC and prog- nostic factors were analyzed. Results （ 1 ） There were more women than men in both groups, but the proportion of men among IBC patients was slightly higher than that among patients with IBCA. The mean clinical symptoms of patients with IBC were signifi- cantly longer than those of IBCA patients （ P〈0. 05 ）. The main clinical manifestations of both groups of patients were abdominal pain and the increased levels of serum CA19-9 and CEA to varying degrees. （2） All the patients with IBC had a median follow -up time of 12. 6 months and a median overall PFS of 5.8 months. （3） Gender, types of tumor, surgical approaches and tumor metastasis were independent risk factors for IBC prognosis. Conclusion Gender, types of tumor （ invasive）, surgical approaches （local excision） and tumor metastasis （yes） are independent risk factors for prognosis of IBC patients.
