Oculus眼表综合分析仪在角膜屈光手术前泪膜功能检测中的有效性和舒适性

摘　　要：目的:探讨传统泪膜测量方法和非侵入性泪膜功能检测方法的相关性、一致性及舒适性。方法:临床病例回顾性研究。选取101例101眼欲行准分子激光角膜屈光手术患者,应用Oculus眼表分析仪测量非侵入性泪膜破裂时间(NIBUT)、下泪河高度(LTMH);SchimerⅠ试验(SⅠt)测量基础泪液分泌、侵入性泪膜破裂时间(fl-BUT)。分别对首检眼的NIBUT与fl-BUT进行相关性、一致性分析,对LTMH与SⅠt进行相关分析。采用视觉模拟评分法(visual analong scale,VAS)对两种检查过程的舒适度进行评价。结果:LTMH与SⅠt呈正相关(rs=0.346,P=0.001)、NIBUT与fl-BUT呈正相关(rs=0.393,P=0.001)。NIBUT与fl-BUT的Bland-Altman图95%一致性界限范围-9.62~14.18。NIBUT检查与fl-BUT检查舒适度评分比较,差异有统计学意义(z=-2.324,P=0.020);LTMH检查与SⅠt检查舒适度评分比较,差异有统计学意义(z=-8.845,P=0.001)。结论:Oculus眼表综合分析仪测量的NIBUT、LTMH值客观有效,LTMH与SⅠt呈良好的正相关,NIBUT与flBUT、SⅠt与LTMH存在正相关,且检查过程较传统方法舒适,能够有效评估角膜屈光手术术前泪膜功能。AIM： To explore correlation,consistency and comfort between traditional tear film examination methods and Oculus Keratograph.METHODS： A retrospective study.Totally 101 cases （101 eyes） were diagnosed myopia and then accepted LASEK （laser epithelial keratomileusis）.Non-invasive tear film break-up time（NIBUT）,lower tear meniscus height（LTMH） were measured with Oculus Keratograph,fluorescein tear film break-up time（fl-BUT） and Schimer Ⅰ test （SⅠt） were performed on all cases.The correlations analysis between NIBUT and fl-BUT,LTMH and SⅠt were performed by Spearman rank correlation,consistency check between NIBUT and fl-BUT by Bland-Altman analysis.Visual analogue scale（VAS） was applied on evaluating the comfort of two kinds of examination methods.RESULTS： LTMH and SⅠt showed positive correlation （rs=0.346,P=0.001）.NIBUT and fl-BUT showed positive correlation （rs=0.393,P=0.001）,95% consistency limits range-9.62 to 14.18 in Bland-Altman Figure.There was significant difference between VAS of NIBUT and VAS of fl-BUT（z=-2.324,P=0.020）.There was significant difference between VAS of LTMH and VAS of SⅠt （z=-8.845,P=0.001）.CONCLUSION： Oculus Keratograph can objectively measure NIBUT and LTMH,and was more comfortable than traditional tear film examination methods.It can effectively assess tear film function before corneal refractive surgery.
