肾素瘤2例报告并文献复习

摘　　要：目的探讨肾素瘤的临床特征、诊治方法及其预后。方法回顾性分析2015年6月‐2016年9月笔者临床遇到的2例肾素瘤患者的临床资料,并结合相关文献,总结归纳肾素瘤的临床特征、诊断治疗及预后情况。结果 1例患者行开放肾部分切除术,1例患者行后腹腔镜下肾部分切除术,术后血压、血钾均恢复正常,术后1周复查血肾素活性均明显下降,术后随访1例6个月、1例15个月计算机断层扫描(CT)检查均未见肿瘤复发。结论肾素瘤是一种良性肿瘤,有较高的肾素活性,主要临床表现为高血压、低血钾等,磁共振成像(MRI)联合CT扫描有助于该病的诊断,肾脏部分切除为有效治疗方法,预后较好。【Objective】To probe into the clinical characteristics, diagnostic methods and prognosis of renin-secreting tumor. 【Methods】The clinical materials of two renin-secreting tumor cases encountered from June 2015 to September 2016 were retrospectively analyzed, and the clinical data, diagnostic treatment and prognostic condition of reninoma were summarized in combination with the related literatures.【Results】One case underwent open partial nephrectomy. Another case was operated on by partial nephrectomy with retroperitoneal laparoscopy, and post-operative blood pressure, blood potassium all returned to normal. Plasma renin activity examined 1 week after operation decreased apparently. One case in the follow-ups of six months, and another of 15 months all had no tumors reoccurred by CT examination.【Conclusion】Reninoma is a benign tumor with higher renin activity, whose clinical manifestation features were high blood pressure and low blood potassium etc. The diagnosis of reninoma can benefit from CT scanning together with MRI. The partial nephrectomy is an effective way of treatment, and also has better prognosis.
