彩色多普勒超声检测脑血管病患者颈动脉硬化的特点及应用价值

摘　　要：目的:观察彩色多普勒超声检测脑血管病患者颈动脉硬化的特点,分析其临床应用价值。方法:选取2011年10月-2016年10月笔者所在医院收治的脑血管病患者70例作为研究对象,再选取同期笔者所在医院体检中心的健康体检者65例作为对照组,分别对两组进行彩色多普勒超声检测,对比两组颈总动脉和颈内动脉的收缩期峰值流速(PSV)、舒张末期流速(EDV)、阻力指数(RI)、搏动指数(PI)、收缩早期加速时间(AT)、IMT增厚、低/等回声斑块、混合回声斑块及强回声斑块。结果:两组颈总动脉PSV、EDV比较差异有统计学意义(P<0.05),RI、PI、AT比较差异无统计学意义(P>0.05);两组颈内动脉PSV、EDV、RI、PI、AT等比较差异均有统计学意义(P<0.05)。两组IMT增厚、低/等回声斑块、混合回声斑块、强回声斑块的比例比较差异均有统计学意义(P<0.05)。结论:彩色多普勒超声检测脑血管病颈动脉硬化具有较高临床诊断意义。Objective：To observe the characteristics of carotid atherosclerosis in patients with cerebrovascular disease by color Doppler ultrasonography and to analyze its clinical application value.Method：From October 2011 to October 2016 in our hospital 70 cases with cerebrovascular disease was as the research object,and then selected 65 cases of healthy persons as the control group from the medical examination center of our hospital.They were all assessed by Doppler ultrasonography.Compared two groups of carotid artery and internal carotid artery peak systolic velocity（PSV）,the end diastolic flow rate（EDV）,pulsatility index（PI）,and resistance index（RI）,early systolic acceleration time（AT）,IMT thickening,low/equal echo plaque,mixed echo plaque and hyperechoic plaque.Result：The two groups had significant difference of the carotid artery PSV,EDV（P〈0.05）,RI,PI,AT had no significant difference between the two groups（P〈0.05）;internal carotid artery of PSV,EDV,RI,PI,AT had significant difference between two groups（P〈0.05）.There was a significant difference in the ratio of IMT thickening,low/equal echo plaque,mixed echo plaque and hyperechoic plaque between the two groups（P〈0.05）.Conclusion：Detection of carotid atherosclerosis in patients with cerebrovascular disease by color Doppler ultrasound has high clinical effect.
