上皮型钙粘附蛋白和层粘连蛋白在妊娠滋养细胞肿瘤的临床价值

摘　　要：目的探讨上皮型钙粘附蛋白(E-cadherin)和层粘连蛋白(Laminin)在妊娠滋养细胞肿瘤(GTN,包括葡萄胎、侵蚀性葡萄、绒毛膜癌)中的表达情况及其临床意义。方法采用免疫组织化学(SP法)检测45例GTN中E-cadherin和Laminin的表达情况。结果在GTN中E-cadherin的阳性表达率和Laminin阳性表达率分别为33.3%(15/45)和60.0%(27/45)。在葡萄胎(HM)、侵蚀性葡萄胎(IM)及绒毛膜癌(CC)中E-cadherin的阳性表达率分别为66.6%、20.0%和13.3%;Laminin的阳性表达率分别为80.0%、60.0%和40.0%。随着GTN恶性程度增加,E-cadherin的阳性表达率及Laminin的阳性表达率均呈逐渐降低趋势。结论 E-cadherin及Laminin在葡萄胎、侵蚀性葡萄胎、绒癌中均有所表达,二者的表达均与妊娠滋养细胞肿瘤的恶性程度有关,对GTN的发生、恶变有一定影响,对E-cadherin与Laminin的检测在临床GTN治疗中抗侵袭和抗转移方面具有重要价值。Objective To analyze the expressions of E-cadherin（E-cad）and laminin（LN）in gestational trophoblastic tumors（GTN）,including hydatidiform mole（HM）,invasive mole（IM）and choriocarcinoma（CC）,and the significance of the E-cad and LN expression was analyzed.Methods The expression of E-cad and LN was detected by immunohistochemistry（IHC）in 45 cases of GTNs.Results In the GTN,the positive expression rates of E-cad and LN were 33.3%（15/45）and 60.0%（27/45）.In HM,IM and CC tissues,the positive expression rate of E-cadherin were 66.6%,20.0% and 13.3% respectively;the positive expression rate of laminin was 80.0%,60.0% and 40.0%,respectively.With the progression of GTN,there was a trend towards gradual reduction.Conclusion Both of E-cad and LN can be expressed in HM,IM and CC.The expression of E-cad and LN was found to be associated with the malignant degree of GTN,and associated with the occurrence and progression of GTN.The detection of E-cad and LN was shown to be of significant value in the investigation of prevention from invasion and metastasis in clinical treatment of the GTN.
