摘 要:目的分析非哺乳期乳腺炎与乳腺癌的超声影像学表现及鉴别价值。方法选取50例非哺乳期乳腺炎与40例乳腺癌患者为研究对象,均行灰阶超声与多普勒超声检查,超声影像学资料与病理资料完整,并经手术或病理活检证实。由2名有经验的超声造影医师根据超声图像进行分析并作出诊断,统计诊断符合率。分析非哺乳期乳腺炎与乳腺癌的超声影像学表现。结果 50例非哺乳期乳腺炎患者超声确诊46例,符合率为92.00%;40例乳腺癌患者超声确诊32例,符合率为80.00%。非哺乳期乳腺炎患者超声表现为肿块形态不规则、边界不清或欠清、低或更低回声斑、微小钙化、腋窝淋巴结结构清晰;乳腺癌患者超声表现为肿块形态不规则、可见毛刺、回声低而不均匀、砂粒样钙化、腋窝淋巴结紊乱甚至消失。非哺乳期乳腺炎、乳腺癌患者血流信号分级分布有统计学意义(P<0.05);且非哺乳期乳腺炎患者收缩期峰值流速(V_(max))和阻力指数(RI)均显著小于乳腺癌患者(P<0.05)。结论非哺乳期乳腺炎与乳腺癌的超声影像学表现具有明显的特点,可指导临床鉴别诊断,但应避免完全依赖超声诊断。Objective To analyze the ultrasound imaging manifestations and diagnostic value in non - lactation mas-titis and breast cancer. Methods 50 cases with non - lactation mastitis and 40 cases with breast cancer were enrolled in the study. All subjects underwent gray - scale and Doppler ultrasound examination. The ultrasound imaging data and pathological data were complete and confirmed by surgery or pathological biopsy. Analysis and diagnosis were performed by two experi-enced ultrasound imaging physicians according to the ultrasound images, while the diagnostic accuracy rate was statistically analyzed. The ultrasound imaging manifestations of non - lactation mastitis and breast cancer were analyzed. Results A-mong 50 cases with non - lactation mastitis, 46 ones were confirmed by the ultrasonograpy, and the coincidence rate was 92. 00% . Among 40 cases with breast cancer, 32 cases were confirmed by ultrasonogrphy and the coincidence rate was 80. 00% . The ultrasound imaging manifestations of non - lactation mastitis included masses with irregular shapes, not clear or unclear boundaries, hypoechoic plaques, microcalcification, and axillary lymph nodes with clear structures. The ultrasound imaging manifestations of breast cancer showed masses with irregular shapes, visible burr, low and inhomogeneous echo, grit calcification, lymph node disorder or even disappearance. There was statistical significance of non - lactation mastitis, blood flow signals in patients with breast cancer grading distribution (P 〈 0. 05 ) ; and non - lactation mastitis patients with peak systolic velocity (Vmax) and resistance index (RI) were significantly lower than those of patients with breast cancer (P 〈 0. 05) . Conclusion The ultrasound imaging manifestations of non - lactation mastitis and breast cancer show so obvious characteristics that they can be used to guide the clinical diagnosis, but entire dependence on the ultrasound diagnosis should be avoided.
