5种指标联合检测对乳腺癌的临床诊断价值

摘　　要：目的:探讨血清β2-微球蛋白(β2-MG)、血浆D-二聚体、糖类抗原125(CA125)、糖类抗原153(CA153)和癌胚抗原(CEA)对乳腺癌的临床诊断价值。方法:对60例乳腺癌患者用电化学发光法检测血清中CA125、CA153和CEA水平,同时采用免疫透射比浊法检测其血清β2-MG和D-二聚体水平,并与乳腺良性疾病组30例和健康对照组60例比较。结果:乳腺癌患者早期血清β2-MG、D-二聚体水平显著高于乳腺良性疾病组,差异有统计学意义(P<0.05),而血清CA153、CA125和CEA与良性疾病组比较无统计学意义(P>0.05);以β2-MG+D-二聚体检测为基础,检测敏感性高达63.33%,特异性为91.66%,与5项联合检测的敏感性比较差异无统计学意义(P>0.05)。结论:血清β2-MG和D-二聚体对乳腺癌患者早期临床诊断优于CA153、CA125和CEA,且具有较好敏感性和特异性,但5种指标联合检测对乳腺癌晚期、转移和治疗监控有较高临床价值。Objective： To investigate the clinical value of serum β2-microspheres（β2-MG）,plasma D-dimer,carbohydrate antigen 125（CA125）,carbohydrate antigen 153（CA153） and carcinoembryonic antigen（CEA） in the diagnosis of breast cancer. Methods： Serum CA125,CA153 and CEA levels in 60 cases of breast cancer patients were detected by chemiluminescence detection, and the immune turbidimetric method was used to detect the serum level of β2-MG and D-dimer,and compared to benign breast disease group of 30 cases and 60 cases of healthy group as control. Results： The serum level of β2-MG and D-dimer in early breast cancer patients was significantly higher than that of benign breast disease group,and the difference was statistically significant（P〈0.05）. There was no significant change in serum CA153,CA125 and CEA in benign disease group（P〉0.05）. Based on the detection of β2-MG＋D-dimer5,the sensitivity of detection was as high as 63. 33%,and the specificity was 91. 66%. There was no significant difference in the 5 combinations（P〉0.05）. Conclusion：Serum β2-MG and D-dimer of patients with breast cancer early diagnosis is better than CA153,CA125 and CEA,and has good sensitivity and specificity,but the combined detection of 5 markers of breast cancer,metastasis and treatment monitor has important clinical practical value.
