运用蛋白质谱技术筛选Ph^+急性淋巴细胞白血病早期中枢复发的脑脊液蛋白标志物

摘　　要：目的研究并探讨蛋白质谱分析技术在筛选Ph阳性急性淋巴细胞白血病患者发生早期中枢神经系统白血病(central nervous system leukemia,CNSL)的脑脊液蛋白标志物。方法选取2011年1月至2016年12月期间经本院和深圳市其他部分医院收治的26例CNSL患者和同期在本院接受治疗的26例病毒性脑膜炎患者为研究对象,采集其在治疗前后以及疾病缓解期等三个不同时期的脑脊液标本,利用SELDI-TOF-MS质谱分析技术对采集到的标本进行检测并建立蛋白质谱,对差异蛋白进行分析。结果比较CNSL组和病毒性脑膜炎组治疗前的蛋白质指纹图谱发现11个存在明显差异的蛋白质峰;同时,CNSL组患者在治疗前和治疗缓解期存在4个有显著差异的蛋白质峰(均P<0.05)。结论利用SELDI-TOF-MS质谱分析技术能够较快找到Ph阳性急性淋巴细胞白血病患者早期发生CNSL的脑脊液蛋白标志物,利于对疾病进行诊断,采取相关治疗手段,以降低患者死亡率和术后复发率,改善预后。Objective To study and explore protein mass spectrometry technology in screening of cerebrospinal fluid protein markers inE arly central nervous system leukemia（CNSL） recurrence of Ph~＋ acute lymphoblastic leukemia.Methods 26 CNSL patients in our hospital from January 2011 to December 2016 were selected as the research object,26 cases of viral meningitis patients treated in our hospital were selected,collected three different periods of remission cerebrospinal fluid specimens before and after treatment,the collected samples were detected by SELDI-TOF-MS mass spectrometry and the protein mass spectrometry was established,the differential proteins were analyzed. Results Compared with CNSL group and viral meningitis group,11 protein peaks were found; At the same time,there were 4 significant protein peaks in patients with CNSL before and after treatment（P0.05）. Conclusions SELDI-TOF-MS mass spectrometry technology can quickly find cerebrospinal fluid protein markers in early central nervous system leukemia（CNSL） recurrence of Ph~＋ acute lymphoblastic leukemia,is conducive to disease diagnosis and treatment to take timely measures to reduce Ph positive acute lymphoblastic leukemia recurrence rate and mortality.
