医务人员健康体检胃镜检查及组织病理结果分析

摘　　要：目的调查了解医务人员健康体检的胃镜及组织病理学检查结果,为上消化道疾病的二级预防提供参考。方法回顾分析2015年8月至2016年12月本院行健康胃镜查体的703例医务人员,统计其性别、年龄、幽门螺杆菌(Helicobacter pylori,HP)感染、食管炎、慢性萎缩性胃炎、胃溃疡等检出情况,对检出疾病人群的性别、年龄、病理特征等进行相关性分析。结果受检者中男女比为1:1.32;受检人群年龄均为40岁以上,检出疾病以食管炎(13.51%)、慢性糜烂型胃炎(12.94%)、胃息肉(12.09%)为主,男性(54.75%)检出居多;医务人员HP总感染率为42.69%,检出消化道溃疡、慢性糜烂性胃炎及萎缩性胃炎的受检者HP感染率较高;病理组织学结果显示胃黏膜组织重度炎症多见于胃溃疡(53.66%),其炎症改变较慢性萎缩性胃炎及胃息肉等严重。胃黏膜萎缩伴肠化生改变多见于慢性萎缩性胃炎及十二指肠球部溃疡。结论医务人员健康查体胃镜检查疾病检出率高,HP感染率高,建议40岁以上医务人员及其他职业人群定期行胃镜健康查体。Objective Investigate gastroscope examination results with pathological analysis in medical staff of healthy physical, to provide reference for secondary prevention of Upper gastrointestinal disease. Method Reviewing 703 medical staff with gastroscopic examination from 2015 to 2016 for physical examination in our hospital, statistical its gender, age, HP infection, oesophagitis, chronic atrophic gastritis, gastric ulcer and the other lesion detection and so forth to do correlation analysis. Result The male to female ratio was 1：1.3. All of patients who received gastroscopy was at 40 years old and above. The majority of diseases were esophagitis （13.51%）, chronic erosive gastritis （12.94%） and gastric polyps （12.09%） respectively. Higher incidence of upper gastrointestinal disease detection in men than women. The HP infection rate of medical staff was 42.69%. Those detected with peptic ulcer,or chronic atrophic gastritis were higher in HP infection rate.Histological results show that the severe inflammation of gastric mucosa tissues were easily found in gastric ulcer （53.66%）, whose inflammation changes is severer than chronic atrophic gastritis,and gastric polyps. The gastric mucosa in atrophy and intestinal metaplasia was found more easily at chronic atrophic gastritis and duodenal bulb ulcers. Conclusion The detection of gastroscopy disease and HP infection rate with medical staff is high.It is necessary for medical staff and Other careers above 40 years old to do gastroscope examination termly.
