摘 要:目的:比较不同热牙胶充填方法在扁圆形根管桩道预备后根尖封闭的应用效果。方法:选取90颗单根管扁圆形离体下颌第一前磨牙,随机分为3组,每组30颗,截冠后统一工作长度为14mm;在根管手术显微镜下运用ProTaper Next镍钛系统进行根管预备,分别采用热垂直加压充填技术、热塑载体牙胶尖充填技术与冷侧压技术进行根管充填,使用1-4#Pesso钻去除根管内牙胶预备桩道,预备深度为9mm;在距根尖孔3mm处作横断面切片,电子显微镜下观察并记录根管内的牙胶、糊剂及空隙占根管横断面的比例,分析不同热牙胶充填方法在扁圆形根管桩道预备后根尖封闭的效果。结果:两种热牙胶充填组在距根尖3mm处,根管桩道预备后根尖封闭效果明显优于冷侧压充填组(P<0.05);热垂直加压重填组稍优于热塑载体牙胶尖充填组,但无统计学差异(P>0.05)。结论:采用热垂直加压充填技术对扁圆形根管进行根管充填,在桩道预备后能获得良好的根尖封闭效果。Objective To compare the effect of apical sealing with the different warm gutter percha obturation in the oblate canal after the post space preparation. Methods To select 90 single oblateroot canal from the first mandibular premolar, crowned the crownand unified the working length of 14mm, randomly divided into three groups of 30. The root canal preparaed with the ProTaper next nickel titanium system under the dental operating microscope. The rootcanal obtured with the warm gutta-percha vertical condensation technique, thermoplastic core-carrier technique andcold lateral condensation technique, then use 1-4#Pesso to remove the gutta percha and prepare the post space, the depth is 9mm. For the cross-sectional slice at 3mm from the apical, recorded the proportion of paste and voids in the root canal section. To analyze the effect of different obturation measure in the apical sealing after the post space preparation of the oblate root canal. Results Two kinds of warm gutta percha obturation group in the apical 3mm, the apical sealing effect is obviously better than cold lateral condensation group (P〈0.05). Warm gutta-percha vertical condensation technique group is slightly better than the tthermoplastic core-carrier technique obturation group, but no statistical difference (P〉0.05). Conclusion Use the warm gutta-percha vertical condensation technology in the oblatecanalobturation could get the well apical sealing effect after the post space preparation.
