摘 要:目的评估Er:YAG激光联合引导组织再生术(GTR)治疗后牙区牙周炎骨缺损的临床疗效。方法选择牙周袋深度≥6 mm,伴垂直型骨吸收的慢性牙周炎患者28例,共60颗患牙,随机分为两组。实验组采用Er:YAG激光联合GTR的方式治疗牙槽骨缺损,对照组采用传统的GTR技术治疗牙槽骨缺损。在基线、3个月和6个月时进行龈沟出血指数(SBI)、牙龈退缩指数(GR)、牙周探诊深度(PPD)、临床附着丧失(CAL)及牙槽骨高度(AH)检查。结果术后两组患牙的各项指标均比基线时明显改善(P<0.05)。与对照组比较,实验组的SBI、PPD和CAL显著减少,AH显著增加(P<0.05);两组的GR无显著差异(P>0.05)。X线检查显示,实验组骨缺损区较对照组获得更多的新骨生成。结论 Er:YAG激光联合GTR治疗后牙区牙周炎骨缺损具有较好的临床效果,牙槽骨再生效果优于单纯GTR。Objective To evaluate the clinical effect of Er:YAG laser combined with guided tissue regeneration (GTR) in the treatment of posterior intrabony defect of periodontitis. Methods 60 posterior teeth in 28 patients with serious periodontal diseases (periodontal pocket depth≥ 6 mm, with vertical bone resorption) were randomized into two groups. The experimental group was treated with Er:YAG laser combined with GTR., and the control group was treated with GTR only. The clinical periodontal index test including sulcus bleeding index (SBI), gingival recession (GR), periodontal probing depth (PPD), clinical attachment loss (CAL) and alveolar bone height (AH) was conducted at baseline, 3 months and 6 months. Results After surgery, all of the clinical periodontal index improved significantly (P 〈0.05). In contrast to the control group, the experimental group had significantly decreased PPD, CAL and SBI, and significantly increased AH (P〈0.05); No statistical difference was seen in GR between the two groups (P〉0.05). The X-ray examination showed that more new bone formation was observed in the experimental group. Conclusions The combined application of Er:.YAG laser and GTR in the treatment of posterior intrabony defect ofperiodontitis has better clinical effect, and the effectiveness of new bone formation is superior to GTR only.
