摘 要:目的研究肾二肽酶基因(Dipeptidase 1,DPEP1)mRNA在左右半结肠癌组织中的表达及其临床意义。方法选取接受结肠癌切除术的腺癌患者80例,以结肠脾曲为界,分为右半结肠癌(right-sided colon cancer,RSCC)和左半结肠癌(left-sided colon cancer,LSCC)组。收集每个患者的肿瘤和自身正常结肠组织标本80对,使用RT-q PCR技术分析DPEP1 mRNA在两组中的表达并分析其临床意义。结果 DPEP1 mRNA在肿瘤组织中表达显著高于正常组织(P<0.001);DPEP1 mRNA在LSCC中的表达显著高于RSCC(P表达指数=0.018);DPEP1 mRNA高表达病人更多见于LSCC(P=0.025);高表达者低分化病人的比例显著高于高分化病人(P=0.009);高表达病人癌相关死亡率显著高于低表达者(P=0.010);DPEP1 mRNA与肿瘤浸润深度T4临界显著相关(P=0.051)。LSCC中DPEP1 mRNA低表达者的中位生存显著优于高表达者(62.8 mo vs.42.0 mo,P=0.010),RSCC高和低表达者之间生存无显著差异(P=0.159)。多因素分析显示,T4侵犯和脉管癌栓是RSCC的独立不良预后因子,T4侵犯、淋巴结转移和脉管癌栓是LSCC的独立不良预后因子,DPEP1高表达与LSCC病人不良预后临界显著相关(P=0.052)。结论 DPEP1 mRNA在LSCC的表达显著高于RSCC,且对两者具有不同的预后意义,其高表达仅对LSCC具有不良预后影响。Objective To explore the mRNA expression of Dipeptidase 1(DPEP1) between the left-sided and right-sided colon adenocarcinomas and its clinical significance. Methods A total of eighty patients, who received radical resection of colon cancer and were confirmed as adenocarcinoma, were identified and divided into left-sided colon cancer(LSCC) and right-sided colon cancer(RSCC)by the boundary point of splenic flexure. Eighty paired specimens were collected from each patient's cancer tissue and itself normal colon tissue. The mRNA gene expression of DPEP1 was detected in two subgroups by the method of real-time quantitative RT-PCR.Results The mRNA expression of DPEP1 in tumor tissue is significantly higher than that in normal colon for all studied patients(P〈0.001). But its expression is markedly higher in LSCC than in RSCC(P = 0.018). The patients with high expression of DPEP1 mRNA are closely related with LSCC(P = 0.025), poor differentiation(P = 0.009)and cancer-related death(P =0.010), but with T4 infiltration of colon wall with marginal significance(P = 0.051). The patients with DPEP1 mRNA low expression survived much longer than those with high expression for the patients with LSCC(62.8 mo vs 42 mo,P = 0.010). However, for the patients with RSCC, DPEP1 expression has no Evident influence on their survival(P = 0.159). In multivariable analysis, T4 infiltration and lymphatic/vascular invasion are the independent poor prognostic factors of RSCC; T4 infiltration, lymph node metastasis and lymphatic/vascular invasion are the independent poor prognostic factors of LSCC. High mRNA expression of DPEP1 shows a non-independent risk factor of LSCC for its marginal significance(P = 0.052). Conclusion The mRNA expression of DPEP1 is significantly differentially expressed between LSCC and RSCC. Its expression affect the survival of RSCC and LSCC differently. It acts as a risk factor of survival only for LSCC.
