摘  要:目的了解淄博市水源性高碘地区孕妇等5类重点人群的碘营养状况以及不同人群之间是否存在差异。方法随机抽取高青县东、西、南、北、中5个个方位的的5个乡镇,采集居民户食用盐和水样进行检测;采集孕妇等5类重点人群的尿样进行尿碘检测,并对8~10岁儿童进行甲状腺B超检查。结果 2015年共采集居民户盐300份,均为无碘盐,无碘盐率为100%;采集水样52份,中位数为225.20μg/L;采集8~10岁儿童、孕妇、哺乳期妇女、婴幼儿和育龄妇女5类重点人群尿样455份,尿碘中位数分别为392.20、168.90、164.80、238.30和375.85μg/L,不同人群尿碘浓度差异有统计学意义(F=6.047,P<0.001=;8~10岁儿童甲肿率为52.71%。结论 2015年高青县水碘中位数显示仍为高碘地区,8~10岁儿童和育龄妇女尿碘水平处于碘过量状态,孕妇、哺乳期妇女和婴幼儿碘营养处于适宜状态,8~10岁儿童甲状腺肿大率处于较高发病水平,重点人群受高碘危害比较严重,应当加强高碘地区不同人群的碘营养状况监测及指导;儿童的尿碘水平不能完全代表孕妇等重点人群的的碘营养状况,应建立孕妇等重点人群自身的碘营养状况监测标准。Objective To investigate the iodine nutritional status among five key populations in high iodine water area and the possible differences among populations in Zibo City. Methods Five townships were randomly selected from east, west, south, north and center in Gaoqing County and the iodine contents in the edible salt and water were analysed. At the same time, urine samples were collected and measured for iodine content. The thyroids of 8-10 years old children's were examined by ultrasound. Results Three hundred family-used edible salt samples were detected. All were non-iodized salt. A total of fifty-two water samples were collected and the median of water iodine was 225.20 μg/L. Four hundred and fifty-five urinary iodine samples among 8-10 years old children, pregnant woman, lactating women, infants and women of childbearing age were detected and the medians of urinary iodine were 392.20, 168.90, 164.80, 238.30 and 375.85 μg/L, respectively. There were statistically significant differences(F=6.047,P〈0.001) in urinary iodine concentrations among different groups. The goiter rate of 8-10 years old children was 52.71%. Conclusion The median of water iodine in 2015 showed that Gaoqing County was still a periodate area. The urinary iodine level of 8-10 years old children and women of childbearing age exceeded the reference level in 2015, and the goiter incidence was high and the health status of key populations was seriously affected by high level of iodine. The surveillance on iodine nutrition status in different populations should be strengthened. Urinary iodine level of children is not a good indicator for the iodine nutritional status of key populations, such as pregnant women. Iodine nutritional status monitoring standard for pregnant women and other key populations should be established.
