旋覆代赭汤合左金丸联合西药治疗肝胃郁热反流性食管炎随机平行对照研究

摘　　要：[目的]观察旋覆代赭汤合左金丸联合西药治疗肝胃郁热反流性食管炎疗效。[方法]使用随机平行对照方法,将90例门诊及住院患者按抛硬币方法随机分为两组。对照组45例雷贝拉唑,20mg(2片)/次,1次/d,晨服;莫沙必利,5mg(1片)/次,3次/d,饭前服用。治疗组45例旋覆代赭汤合左金丸(旋覆花包煎15g,代赭石先煎5g,党参10g,炙甘草、清半夏各15g,生姜25g,黄连18g,吴茱萸3g,大枣4枚;嗳气较重加柿蒂15g,丁香10g;反酸较重加锻瓦楞子先煎15g,海螵蛸20g),1剂/d,水煎300m L,早晚口服;雷贝拉唑与莫沙必利治疗同对照组。连续治疗4周为1疗程。观测临床症状、内镜检查总积分、RDQ积分、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈12例,显效26例,有效5例,无效2例,总有效率95.55%;对照组痊愈10例,显效18例,有效9例,无效8例,总有效率82.22%;治疗组疗效优于对照组(P<0.05)。内镜检查总积分、RDQ积分两组均有改善(P<0.05),治疗组改善优于对照组(P<0.01,P<0.05)。[结论]旋覆代赭汤合左金丸联合西药治疗肝胃郁热反流性食管炎,疗效满意,无严重不良反应,值得推广。[Objective] To observe Xuanfudaizhe decoction of Zuojin Pill Combined with western medicine in the treatment of liver stomach heat reflux esophagitis curative effect. [Method] Using random parallel control method, 90 cases of outpatient and hospitalized patients by coin method were randomly divided into two groups. The control group of 45 cases with ray Bella, 20mg （2）/time, 1 times/d, taken in the morning; mosapride, 5mg （1）/time, 3 times/d and before meals. The treatment group of 45 cases of Xuanfudaizhe decoction combined with Zuojin pills （inula bag decoction 15g, daizhushi decocted 5g, dangshen 10g, licorice root, rhizoma pinelliae 15g, ginger 25g, coptis evodia 18g, 3g, jujube 4 pieces; belching heavy plus Diospyros kaki 15g, clove 10g acid; heavy forging with concha arcae decocted 15g, cuttlebone, 1） 20g agent/d, 300mL oral decoction, sooner or later; ray bella met with mosapride treatment with the control group. 4 weeks ofcontinuous treatment for 1 course of treatment. Observation of clinical symptoms, endoscopic examination total score, RDQ score, adverse reaction. Continuous treatment of 2 courses and the curative effect. [Results] The treatment group 12 cases were cured, 26 cases markedly effective, 5 cases effective, 2 cases ineffective, the total efficiency of 95.55%; control group 10 cases were cured, 18 cases markedly effective, 9 cases effective, 8 cases ineffective, the total efficiency of 82.22%; the treatment group than the control group （P〈0.05）. Total endoscopy integral control group, treatment group was better than that of RDQ integral （P〈0.05, P〈0.01）. [Conclusion] Xuanfudaizhe decoction of Zuojin Pill Combined with western medicine in the treatment of liver stomach heat reflux esophagitis, satisfactory curative effect, no serious adverse reactions, worthy of promotion.
