摘 要:目的:采用CT评估正常通气猪肺射频与微波消融的效果。方法:健康家猪6只,每侧肺各布置两个消融区,在CT引导下左肺行经皮射频消融,右肺行经皮微波消融,共消融10min,消融过程中每分钟进行1次CT扫描,获得不同时间点消融灶的图像。测量并比较各不同消融方法及不同时间点消融灶的最大纵径、最大横径、横截面积、体积和横纵径比。结果:时间与消融方式均会对消融灶的大小产生影响(P<0.05)。在各个时间点上,微波消融灶的最大纵径、最大横径、横截面积、体积均大于射频。消融10min后,射频与微波消融灶的最大纵径分别为(4.23±0.35)cm、(5.34±0.39)cm,最大横径分别为(3.09±0.18)cm、(3.83±0.44)cm,消融灶横截面积S分别为(10.27±1.24)cm2和(16.15±2.79)cm2,体积V分别为(21.25±3.60)cm3和(41.96±12.02)cm3,差异均具有统计学意义(P<0.001)。结论:微波和射频均是安全的微创疗法,在正常通气肺组织中的消融范围及形状存在一定差异,了解其特性有利于两种技术的合理应用。Objective:The purpose of this study was to evaluate the effect of radiofrequency ablation (RFA) and microwave ablation (MWA) on models of normal ventilation pig lungs. Methods: Two ablation regions on the left lungs of 6 healthy pigs were performed by RFA and the right lungs by MWA for 10 minutes under CT guidance. CT scan was per-formed every other one minute during ablation. CT images of the ablation zone were obtained at different time point. Maximum longitudinal diameter (MLD), maximum transverse diameter (MTD), horizontal cross--section area ( HCSA), volume (V) ,and the ratio of transverse and longitude (L/T) of the ablation areas were measured and compared between the two methods of different time points. Results:The time duration and method of ablation significantly affected the size of ablation zone (P〈0.05). At each time point,the MLD,MTD,HCSA and V of the MWA were all greater than RFA. MLD of RFA and MWA was (4.23±0.35)em and (5. 34±0. 39)cm;MTD was (3. 09±0. 18)cm and (3. 83±0. 44) cm; HCSA was (10.27±1.24)cm2 and (16.15±2.79)cm2 iV was (21.25±3.60)cm3 and (41.96±12.02)cm3 10 minutes after ablation. Compared with RFA,the differences were all statistically different (P〈0. 001). Conclusion: RFA and MWA are safe and minimally invasive treatments. The ablation zone and shape of RFA and MWA are different. It is helpful to apply the two approaches by understanding their characteristics.
