摘 要:目的探讨同伴支持教育在1型糖尿病患者中的应用效果。方法将68例1型糖尿病患者随机分成实验组及对照组,每组34例。实验组接受同伴支持教育,对照组接受常规健康教育。比较两组干预前、干预后3个月、6个月时的空腹血糖、餐后2 h血糖、糖化血红蛋白、中文版糖尿病痛苦量表得分及两组参与维持阶段教育次数。结果两组的餐后2 h血糖、糖化血红蛋白的组间效应及时间效应差异均有统计学意义,两组的空腹血糖的时间效应差异有统计学意义,两组的餐后2 h血糖、空腹血糖和糖化血红蛋白的干预与时间因素存在交互效应。两组的糖尿病痛苦量表总分及与生活规律相关、与情感相关、与人际关系相关维度得分的组间效应差异有统计学意义(P<0.05);两组的糖尿病痛苦量表总分和各维度得分的时间效应(P<0.01)差异有统计学意义,两组的糖尿病痛苦量表总分和各维度得分的干预与时间因素存在交互效应。结论同伴支持教育模式能够显著提高1型糖尿病患者的血糖管理能力,改善其心理状态,提高其参与健康教育的主动性。Objective To evaluate the effects of a peer support education program in patients with type I dia- betes mellitus. Methods A total of 68 patients with type 1 diabetes mellitus were enrolled and randomized into the experimental group and the control group,with 34 cases in each group. Both groups received diabetes health e- ducation for six months; the experimental group was given peer support education,and the control group was given routine health education. Fasting blood glueose,2-hour postprandial blood glucose,HbAlc,scores of China Diabetes Stress Scale,and perceived number of times for attending education programs were compared between two groups before intervention,and 3,6 months after intervention. Results The significant differences were found in HbAlc and 2-hour postprandial blood glucose between two groups and among time. Intervention and time interactively af- fected fasting blood glucose,2-hour postprandial blood glucose and HbAle of two groups significantly. There were significant differences in total score,the scores of regular life,emotion and human relationship dimensions of Dia- betes Stress Scale between two groups (P〈0.05). Among time the differences can be found in the total score and the score of each dimension (P〈0.01). Intervention and time interactively affected the total score and the scores of each dimension of Diabetes Stress Scale in two groups. Conclusion Peer support education program can promote blood glucose control in type 1 diabetes mellitus,improve their psychological status and increase their enthusiasm for being involved in disease management.
