微血管减压术治疗三叉神经痛临床效果的影响因素分析

摘　　要：目的探究微血管减压术治疗三叉神经痛临床疗效的影响因素。方法选取2015年1月~2016年12月我院行微血管减压术治疗的43例三叉神经痛患者为研究对象,对所有患者进行为期4个月随访,根据是否复发进行分组,对比复发组和未复发组患者的年龄、性别、类型、病变部位、病程、受压部位、影像学结果、有无责任血管等因素差异,对有明显差异的因素进一步行logistics多元回归分析。结果本次43例患者中失访2例,最终纳入研究41例,其中复发12例,占29.3%;单因素分析结果显示,年龄、类型、影像学结果、有无责任血管是影响疗效的主要因素(P<0.05);多元Logistic回归分析结果显示,不典型三叉神经痛、影像学检查有血管压迫、术中发现责任血管是导致微血管减压术治疗三叉神经痛疗效降低的独立危险因素(P<0.05)。结论不典型三叉神经痛、影像学检查有血管压迫、术中发现责任血管是导致微血管减压术治疗三叉神经痛疗效降低的独立危险因素,应当针对这些因素做好干预措施,改善患者的预后。Objective To explore the influencing factors of microvascular decompression in the treatment of trigeminal neuralgia. Methods A total of 43 patients with trigeminal neuralgia were treated with mierovascular decompression in our hospital from January 2015 to December 2016. All patients were followed up for 4 months.The patients were followed up for recurrence and relapse.The factors such as age,sex,type,lesion location,course of disease,compression site, imaging outcome,and whether the responsibility of blood vessels and other factors were compared between two groups, among which with significant differences in the factors was given multiple regression analysis.Results Of the 43 patients,2 cases were lost, and 41 cases were included in the study.Among them,12 cases （29.3%） were recurrent. The resuits of univariate analysis showed that age,type,imaging results were the main factors that affected the curative effect （P〈0.05）;multivariate Logistic regression analysis showed that atypical trigeminal neuralgia,angiographic examination with vascular compression,intraoperative findings of the responsibility of blood vessels were the results of microvascular decompression in the treatment of trigeminal neuralgia to reduce the efficacy of independent risk factors（P〈0.05）.Conclusion Atypical trigeminal neuralgia,imaging examination with vascular compression,intraoperative findings of the responsibility of blood vessels are the results of microvascular decompression in the treatment of trigeminal neuralgia to reduce the efficacy of independent risk factors,these factors should be effective intervention measures to improve the prognosis of patients.
