不同屈光手术对双眼视功能的影响研究

摘　　要：目的探讨近视患者行LASIK与LASEK两种手术方式对双眼视功能(双眼调节幅度、AC/A及调节灵敏度)的影响,以寻求导致LASIK与LASEK术后早期近距离阅读困难和视疲劳的原因,同时为近视眼患者术前手术方式的选择及手术量的设计提供另一参考依据。方法临床病例对照研究。选取来我院行LASIK术60例(120只眼)近视眼患者作为LASIK组,选取例来我院行LASEK术60例(120只眼)近视眼患者作为LASEK组,LASIK组和LASEK组所有病例近视度数<-6.00 D,年龄<30岁。观察LASIK组和LASEK组所有患者术前及术后1周、1个月、3个月的双眼调节幅度、AC/A及调节灵敏度的变化,分别进行统计学比较和分析。结果与术前戴镜比较,LASIK组和LASEK组在手术后1周、1个月的双眼调节幅度、AC/A及调节灵敏度差异均有统计学意义(P<0.05)。两组术前双眼调节幅度、AC/A及调节灵敏度比较差异无统计学意义(P均>0.05);两组在术后1周双眼调节幅度、AC/A及调节灵敏度比较差异均有统计学意义(P<0.05),在术后1个月、3个月双眼调节幅度、AC/A及调节灵敏度比较差异无统计学意义(P均>0.05)。结论 LASIK和LASEK术后双眼调节幅度、AC/A及调节灵敏度均呈一过性的降低,然后逐渐恢复,与早期近距离阅读困难和视疲劳有关。Objective To investigate the effects of LASIK and LASEK, two different refractive surgical methods, on the binocular functions. To seek possible causes for reading difficulties and astbenopia after surgeries and to provide a reference for surgery method design. Methods Clinical case-control study. 40 patients （ 80 eyes ） who received LASIK and another 40 patients （ 80 eyes ） who received LASEK in our hospital were enrolled. All patients were younger than 30 years and had myopia that was greater than -6. 00 D. Amplitude of accommodation, AC/A ratio and accommodation sensitivity were observed before and after surgery at 1 week, 1 month, and 3 months. Results Both LASIK and LASEK resulted in reduced accommodation amplitude, AC/A ratio and accommodation sensitvity at 1 week and 1 month （ P 〈 0. 05 ）. There was no significant difference between the two groups except at 1 week after surgery. Conclusions LASIK and LASEK will cause temporary, early post-operative reduction in binocular functions such as accommodation amplitude, AC/A ratio and accommodation sensitivity. These findings may be related to reading difficulties and asthenopia early after the refractive sur- geries.
