摘  要:目的探讨Nd:YAG激光治疗白内障术后继发睫状环阻滞性青光眼的效果及安全性。方法回顾性分析我院17例白内障术后继发睫状环阻滞性青光眼病例,其中单纯白内障术后6例,白内障联合青光眼术后11例,采取Nd:YAG激光切开晶状体后囊及前部玻璃体,使玻璃体水囊与前房沟通。3只眼接受3次激光治疗,5只眼接受2次激光治疗,9只眼接受1次激光治疗。检查患者术前及术后1、3 d、1周,1、3个月的眼压及前房深度,进行统计学分析。结果术后随访3个月,术后1 d眼压明显下降,前房加深,较术前有统计学差异;术后3 d眼部情况趋于稳定,与术后1周、1个月及3个月无统计学差异。结论对白内障术后继发睫状环阻滞性青光眼,及时采取Nd:YAG激光治疗,沟通前房与玻璃体水囊,重建房水正常流出通道,可有效缓解睫状环阻滞性青光眼,降低眼压,保护视功能。Objective To study the efficacy and safety of Nd:YAG laser treatment for malignant glaucoma after cataract surgery. Methods Retrospective analysis of 17 cases patients with malignant glaucoma after cataract surgery. 6 cases of cataract surgery and 11 cases of cataract combined with glaucoma surgery were analyzed. YAG laser was used to penetrate the posterior capsule of the lens and anterior vitreous, allowing aqueous fluid to flow into anterior chamber. 3 eyes were treated 3 times, 5 eyes were treated twice, and 9 eyes were treated once. Intraocular pressure and anterior chamber depth at 1 day, 3 days, 7 days, 1 month and 3 months were observed. Results The follow-up period were 3 months. In- traoeular pressure significantly decreased and anterior chamber depth increased significantly on the first day after therapy. Ocular condition tended to become stable after 3 days. Conclusions Treating ciliary block glaucoma after cataract surgery with Nd :~AG laser timely can drain the vitreous aqueous fluid into anterior chamber and rebuild normal outflow channels. This treatment can effectively alleviate the ciliary block glaucoma, decrease intraocular pressure, and protect the visual function.
