脂质体阿霉素联合奥沙利铂对晚期胃癌患者血清水平及免疫功能的影响

摘　　要：目的探讨脂质体阿霉素联合奥沙利铂对晚期胃癌患者血清CA19-9、CEA、CA125、CA72-4、EGFR水平及免疫功能的影响,分析脂质体阿霉素在晚期胃癌治疗中的作用效果。方法选择65例晚期胃癌患者作为观察对象,并根据随机数字法分为试验组和对照组。对照组给予奥沙利铂联合替吉奥化疗方案,试验组给予脂质体阿霉素联合奥沙利铂化疗方案,比较2组的临床疗效、化疗前后的血清CA19-9、CEA、CA125、CA72-4、EGFR及CD3^+、CD4^+、CD8^+等免疫指标的水平变化。结果对照组的临床总有效率为34.3%,试验组的临床总有效率为46.7%,试验组的近期疗效好于对照组(P<0.05)。经治疗后,2组的血清CA19-9、CEA、CA125、CA72-4、EGFR水平均较治疗前显著降低(P<0.05),且试验组的各项指标显著低于对照组(P<0.05)。经治疗后,2组患者的CD3^+、CD4^+、CD8^+水平均显著高于治疗前(P<0.05),且试验组的各项指标均高于对照组(P<0.05)。结论脂质体阿霉素联合奥沙利铂治疗晚期胃癌的近期疗效可靠,能有效抑制CA19-9、CEA、CA125、CA72-4、EGFR等肿瘤标志物的表达,提高CD3^+、CD4^+、CD8^+等免疫指标在晚期胃癌患者体内的含量,是1种较优的晚期胃癌化疗方案。Objective To investigate the effect of liposomal doxorubicin combined with oxaliplatin for advanced gastric cancer patients serum CA19-9, CEA ,CA125, CA72-4 ,EGFR levels and immune function,analysis of the effect of liposomal doxo- rubicin in the treatment of advanced gastric cancer. Methods 65 patients with advanced gastric cancer were selected as observa- tion objects and were divided into the experimental group and the control group according to random number method. The control group was treated with oxaliplatin combined with S-1 chemotherapy, the experimental group was given liposomal doxorubicin com- bined with oxaliplatin chemotherapy before and after chemotherapy, changes of clinical efficacy of the 2 groups, the serum levels of CA19-9, CEA, CA125, CA72-4 ,EGFR and CD3 ＋ , CD4 ＋ , CD8 ＋ and other immune indexes. Results The total effective rate of the control group was 34.3%0 ,the total effective rate of the experimental group was 46.7% ,and the short-term curative effect of the exper/mental group was better than that of the control group （P 〈 0.05 ）. After the treatment, serum CA19-9, CEA, CA125, CA72-4 and EGFR levels of the 2 groups were significantly lower than those before treatment（ P 〈 0.05 ）, and the experimental group were significantly lower than the control group （P 〈 O. 05 ） ; after the treatment, the 2 groups of patients with CD3 ＋ , CD4 ＋ and CD8 ＋ levels were significantly higher than those before treatment （ P 〈 0.05 ） , and the indicators in the experimental group were higher than those of the control group（ P 〈 0.05 ）. Conclusion The short term curative effect of liposomal doxorubicin com- bined with oxaliplatin in the treatment of advanced gastric cancer is reliable, can effectively inhibit tumor CA19-9, CEA, CA125, CA72-4 and EGFR markers, CD3 ＋ , CD4 ＋ , CD8 ＋ increased the content of immune indexes in patients with advanced gastric canc- er,is a better scheme of chemotherapy for advanced gastric cancer.
