围手术期营养支持对结直肠癌患者肿瘤细胞cyclin D1表达及复发转移的影响

摘　　要：目的探讨结直肠癌患者围术期营养支持后对肿瘤细胞cyclinD1表达及复发转移的影响效果。方法选择148例结直肠癌患者,按照患者是否进行围术期营养支持的意愿,将患者分为营养支持观察组和非营养支持对照组,观察组78例,对照组70例。围术期营养支持为我院配制的营养液,与术前一周及术后5~14 d对观察组患者进行全胃肠外营养,对照组患者进行常规的护理。采用SP免疫组织化学法检测cyclin D1的表达,用TUNEL方法检测细胞的凋亡,细胞核抗原免疫组织化学染色检测细胞增殖,对两组患者进行对比分析。结果经围术期营养支持的患者肿瘤细胞cyclin D1的阳性表达率为(35.12±5.09)%,凋亡指数为(52.67±5.43)%,细胞增殖指数为(7.35±2.87)%;对照组的阳性表达率为(37.50±6.12)%,凋亡指数为(56.17±6.38)%,细胞增殖指数为(8.42±3.67)%;两组比较无统计学差异(P>0.05)。两组患者术后肿瘤的复发率分别为15.38%(12/78)、15.71%(11/70),两组比较无显著性差异(P>0.05)。结论围术期是否对患者进行营养支持对结直肠癌患者肿瘤细胞cyclin D1的表达无显著的影响,同时不会促进肿瘤的复发和转移。Objective To evaluate the effects of perioperative total parenteral nutrition on cyclinD1 ,recurrence and me- tastasis of colorectal cancer. Methods A total of 148 cases of colorectal cancer patients, according to whether receive periopera- tive nutritional support ,were divided into the observation group（ nutritional support ）and the control group（ non-nutritional support） , the observation group had 78 patients and the control group had 70 cases. Perioperative nugritional support for the prepara- tion of nutrient solution for the hospital, week preoperative and postoperative 5 to 14d on the observation group with total patenteral nutrition ,routine care for control group. By SP immunohistochemical method to detect the expression of cyclinDl, and apoptosis u- sing the TUNEL method, cell nuclear antigen immunohistochemical staining was used to detect cell proliferation, the 2 groups of patients were analyzed. Results After perioperative nutrition support,the expression rates of cyclinD1 ,PT and AI in the 2 groups were（35.12 ±5.09）%, （37.50 ±6.12）% ;（52.67 ±5.43）% ,（56.17±6.38）% ;（7.35±2.87）%, （8.42 ±3.67）% ;there were no significant difference in PI, AI and expression of cyclinD1 （P 〉 0.05 ）. The recurrence rates in the 2 groups were 15.38% （12/78） and 15.71% （11/70）. Conclusion Perioperative TPN can not promote proliferation and apoptosis of carcinoma cells, and has no significant impact on the expression of cyclin D1 ,recurrence or metastasis of colorectal cancer.
