纳米炭混悬注射液示踪前哨淋巴结对宫颈癌淋巴结清扫及病理结果的影响

摘　　要：目的探究纳米碳混悬注射液在宫颈癌手术治疗中对前哨淋巴结的示踪以及对淋巴结清扫及病理结果的影响。方法选取宫颈癌患者58例,随机分为示踪组和对照组。示踪组进行纳米碳混悬溶液注射,两组患者均行宫颈癌根治术(广泛子宫全切除术+双侧盆腔淋巴及清除术)。比较两组患者的淋巴结清扫情况和病理组织学检查结果。结果术中肉眼观察示踪组患者的盆腔淋巴结均有黑染,示踪成功率为100%;对前哨淋巴结(包括髂内和闭孔淋巴结)的示踪成功率为93.10%;纳米碳混悬溶液对前哨淋巴结的黑染率比对其他部位(髂外和髂总)淋巴结的黑染率高(P<0.05)。两组患者手术时长、术中出血量以及淋巴结清扫数比较无显著差别(P>0.05)。术后病理结果显示:示踪组4例有淋巴结转移,均为前哨淋巴结,其中3例有黑染;对照组2例患者淋巴结转移,均为前哨淋巴结。结论纳米碳混悬溶液能有效示踪宫颈癌前哨淋巴结,是1种安全高效的淋巴结示踪剂。Objective To explore the effect of nano carbon suspension injection in the treatment of cervical cancer on the sentinel lymph node and the situation of lymph node dissection. Methods 58 cases of cervical cancer were selected and ran- domly divided into 2 groups：the tracer group and the control group. The tracer group was injected with nano carbon suspension so- lution, and 2 groups of patients were treated with radical hysterectomy （ extensive hysterectomy and bilateral pelvic lymph nodes and removal）. The results of lymph node dissection and histological examination were compared between the 2 groups, and the effect of nano carbon suspension was evaluated in the lymph node of cervical cancer. Results Intraoperative visual observation tracer group of patients with pelvic lymph nodes were stained black, tracing the success rate was 100%. The sentinel lymph node （including internal iliac and obturator lymph nodes） of the tracer success rate was 93.10% ;nano carbon mixed suspension solu- tion of sentinel lymph node of the black dye number than in other parts of the body （ iliac and external iliac total） lymph nodes stained black number（P 〈 0.05）. Two groups of patients with operation time,intraoperative bleeding and lymph nodes compared with no significant difference（P 〉0.05 ）. The postoperative pathological results showed that 4 cases had lymph node metastasis, which were located in sentinel lymph nodes, and 3 cases were black stained ;2 patients of the control group with lymph node me- tastasis,were from sentinel lymph node. Conclusion Nano carbon suspension solution can effectively trace the sentinel lymph node of cervical cancer, and it is a safe and efficient lymph node tracer.
