探析粪乳铁蛋白联合血清C反应蛋白在活动性溃疡性结肠炎诊断中的意义

摘　　要：目的探析粪乳铁蛋白联合血清C反应蛋白在判断活动性溃疡性结肠炎的临床诊断价值。方法 90例溃疡性结肠炎患者作为研究对象,根据黏膜活检组织学检查结合Sutherland DAI评分标准分为溃疡性结肠炎活动期组(A1组)和溃疡性结肠炎缓解期组(A2组),各45例;同期选择健康体检者45例作为A3组,对A1、A2、A3组的粪乳铁蛋白及血清C反应蛋白检测结果加以回顾性分析。结果 A1组粪乳铁蛋白为(387.71±16.35)μg/L,C反应蛋白为(31.99±0.95)mg/L,A2组分别为(196.93±24.04)μg/L、(17.64±1.77)mg/L,A3组分别为(17.94±9.63)μg/L、(3.50±0.23)mg/L;A1组粪乳铁蛋白及血清C反应蛋白高于A2组和A3组,A2组高于A3组,差异均有统计学意义(P<0.01)。粪乳铁蛋白联合血清C反应蛋白对活动性溃疡性结肠炎诊断的敏感度为99.7%,特异度为99.0%。结论粪乳铁蛋白联合血清C反应蛋白对活动性溃疡性结肠炎的诊断值得在临床上推广。Objective To explore and analyze the clinical value of fecal lactoferrin and serum C reactive protein in the diagnosis of active ulcerative colitis. Methods A total of 90 ulcerative colitis patients as study subjects were divided by histological examination of mucosa biopsy and Sutherland DAI scoring standard into ulcerative colitis active group（group A1）and ulcerative colitis remission group（group A2）, with 45 cases in each group. Concurrent 45 physical examination people were taken as group A3. Retrospective analysis were made on fecal lactoferrin and serum C reactive protein test results in group A1, A2 and A3. Results Group A1 had fecal lactoferrin as（387.71±16.35） μg/L, C reactive protein as（31.99±0.95） mg/L, which were respectively as（196.93±24.04） μg/L,（17.64±1.77） mg/L in group A2, and（17.94±9.63） μg/L,（3.50±0.23） mg/L in group A3. Group A1 had higher fecal lactoferrin and serum C reactive protein than group A2 and group A3, and group A2 was higher than group A3. Their difference had statistical significance（P〈0.01）. The fecal lactoferrin and serum C reactive protein had sensitivity as 99.7% and specificity as 99% in diagnosis of active ulcerative colitis. Conclusion The diagnosis of active ulcerative colitis with fecal lactoferrin and serum C reactive protein is worthy of clinical promotion.
